Emergency CT: experience in a general hospital |
This retrospective study was performed to precise indications of emergency CT in ageneral hospital over a 30 months period. We tried to determine, with help of priorstudies, the indications for CT of the brain in the management of acute meningitis, acuteheadache, and in the management of head injury. In acute meningitis, there is no evidenceto recommend CT of the brain before lumbar puncture, except to identify patients atincreased risk of cerebral herniation. The imaging study of choice in subarachnoidhemorrhage is non enhanced CT scan. This exam has to be performed in case of acuteheadache. The CT evaluation of patients with minor head injury remains controversial.
Les urgences motivant la réalisation d'un scanner dans un centre hospitalier généralsont diverses. Nous avons examiné rétrospectivement 30 mois d'activité scanographiqued'urgence et essayé, à l'aide de la littérature, de préciser les indications duscanner cérébral en urgence après traumatisme crânien et dans le cadre descéphalées, associées ou non à un syndrome méningé et/ou à une hyperthermie. En casde suspicion d'hémorragie méningée, le scanner est le meilleur examen à réaliser enurgence. En cas de méningite, sa seule indication est d'éliminer une contre-indicationà la ponction lombaire, et son utilité est discutée dans la littérature. Laréalisation systématique d'un scanner en cas de traumatisme crânien mineur a étéproposée, mais reste très discutée.
