Method |
Even though diagnosis of carpal tunnel syndrome is mainly based on clinical findings, other examinations are often useful for confirmation and management. The most useful of these examinations is EMG. However, EMG may be inconclusive and MRI may then be helpful. The indications for MRI in patients with carpal tunnel syndrome will be reviewed.
20 patients with a total of 33 clinically suspected cases of carpal tunnel syndrome (CTS) underwent EMG and MRI evaluation. Clinical and EMG findings identified three groups of patients based on degree of deficit: mild, moderate, and severe. The following structures were evaluated at MRI: median nerve, retinaculum, retrotendinous fat, flexor tendons, thenar space, and muscles and bones of the wrist. Surgery was performed for 19 wrists.
Results |
Only retinacular bowing and increased T2W signal intensity within the median nerve were significantly related to the diagnosis of CTS (sensitivity of 70% and 57% respectively). Retinacular bowing indicates increased "pressure" within the compartment (mechanical compression of the nerve) and increasedc T2W signal of the median nerve indicates nerve suffering. These findings correlated well with more severe cases based on clinical and EMG findings.
Conclusion |
In cases where there is discordance between clinical and EMG findings, MRI is helpful to identify patients who would benefit from surgical intervention.

Le diagnostic de syndrome du canal carpien est avant tout clinique. Les examens complémentaires apportent une aide au diagnostic positif et à la décision thérapeutique. La méthode la plus utilisée en pratique courante est l'électroneurophysiologie (EMG). Ses limites conduisent parfois à utiliser l'imagerie par résonance magnétique dont nous voulons préciser les indications.
Vingt patients porteurs de trente trois syndromes du canal carpien cliniquement suspectés ont été explorés en EMG et IRM. L'examen clinique et l'EMG ont permis de distinguer trois groupes selon le degré d'atteinte : discret, modéré, sévère. Le nerf médian, le retinaculum, la graisse rétrotendineuse, les tendons fléchisseurs, la loge thénar, les structures osseuses et musculaires ont été analysés en IRM (quarante-deux canaux carpiens), dix neufs canaux carpiens ont été opérés.
Résultats |
Seules la convexité du ligament annulaire du carpe (CLAC) et l'existence d'un hypersignal en T2 du nerf médian ont été retenues (sensibilité suffisante, respectivement 70 et 57 %). La CLAC rend compte de l'hyperpression intracompartimentale et donc de la compression mécanique du nerf. L'hypersignal T2 traduit la souffrance fonctionnelle du nerf. Elle est corrélée aux formes sévères cliniques et/ou électromyographiques.
Conclusion |
En cas de discordance entre clinique et EMG, la présence d'un hypersignal en T2 sélectionne les patients susceptibles de bénéficier d'une intervention.
