Purpose. To assess the value of ventilation-perfusion scintigraphy after an initial helical CT with absence of pulmonary embolism (PE).
Material and methods. Twenty eight consecutive patients with clinical suspicion of pulmonary embolism underwent helical CT scan to look for an intra-luminal clot. When the CT-scan was positive for PE, diagnosis was retained. If the result was negative, a ventilation-perfusion scintigraphy was performed. If the result was also negative, diagnosis of PE was excluded. In case of discrepancy, a pulmonary angiography was performed and used as gold-standard.
Results. Twenty-one patients showed PE at CT-angio and 7 did not. Two of these 7 had normal or low probability scintigraphy, excluding diagnosis of PE. One had intermediate V/P scan and 4 had high probability V/P scan. For these 5 discrepancies, none of them had PE on angiography.
Conclusion. In this population, scintigraphy was not appropriate after a negative CT-angio for PE. In case of persistent doubt after a normal initial helical CT, pulmonary angiography is required
But. Évaluer l'intérêt d'une scintigraphie pulmonaire après un résultat tomodensitométrique négatif pour la recherche d'embolie pulmonaire (EP).
Matériel et méthodes. Vingt-huit patients consécutifs avec suspicion clinique d'EP ont été programmés au scanner hélicoïdal pour rechercher une migration embolique. Lorsque le scanner était positif, le diagnostic d'EP était retenu. En cas de résultat négatif, une scintigraphie de ventilation-perfusion était réalisée : si la scintigraphie était négative, le diagnostic d'EP était exclu. En cas de discordance entre le scanner et la scintigraphie, une angiographie servait de référence.
Résultats. Vingt et un patients sur 28 avaient une EP au scanner et 7 patients ne présentaient pas de signes d'EP. Sur ces 7 patients, 2 avaient une scintigraphie normale ou de faible probabilité (excluant le diagnostic), 1 avait une probabilité intermédiaire et 4 une haute probabilité d'EP à la scintigraphie. Sur les 5 cas discordants, aucun n'avait d'EP à l'angiographie.
Conclusion. Dans la population étudiée, la scintigraphie n'avait pas d'intérêt en seconde intention si le scanner initial était normal. En cas de doute persistant après un angioscanner initial normal, une angiographie est recommandée.
