La dissection spontanée de l'artère carotide est une cause rare d'accident vasculaire cérébral. Un homme de 54 ans est hospitalisé pour accident ischémique transitoire. L'échographie-doppler des artères cervicales révèle les signes indirects d'une dissection de l'artère carotide interne droite dans son segment post-bulbaire. L'angiographie par résonance magnétique montre une dissection de l'artère carotide interne droite et l'infarcissement de l'hémisphère cérébral droit. L'angiographie confirme le diagnostic. Le traitement anti-coagulant débuté dès la suspicion diagnostique, est interrompu compte tenu de l'importance du ramollissement cérébral. L'évolution est défavorable avec la persistance d'une hémiplégie séquellaire. (J Mal Vasc 2006 ; 31 : 146-150)
Spontaneous dissection of the carotid artery is a rare cause of stroke. A 54-year-old man was hospitalized for transient ischemic attack. Duplex ultrasound of cervical arteries showed indirect signs of a dissection of the carotid. Magnetic resonance angiography showed a dissection of the post bulbar segment of the right carotid artery and a right ischemic stroke. Angiography confirmed the diagnosis. The anti-coagulant treatment initialized as soon as the diagnosis of dissection was suspected, was stopped because of a massive cerebral infarct. The course was unfavorable with a persistent hemiplegia. (J Mal Vasc 2006; 31: 146-150)
