Prevalence and factors associated with iron deficiency in patients with impaired ejection fraction heart failure at Central Army Hospital-Algiers - 25/12/18

Introduction |
Iron deficiency is a common comorbidity in patients with cardiac failure with altered ejection fraction. Studies report a prevalence of 30–50% in this population. Its deleterious functional and prognostic role is now well established even in the absence of anemia.
The objective of our study was to evaluate the prevalence and factors associated with iron deficiency in heart failure with impaired ejection fraction.
Patients and methods |
Descriptive and prospective cross-sectional study conducted in patients with heart failure with altered ejection fraction (LVEF<40%) hospitalized in the cardiology department of the Central Hospital of the Army of Algiers from 01/04/2017 to 31/01/2018.
In addition to the collection of clinical, biological and echocardiographic parameters, a martial assessment including serum iron, ferritinemia, TIBC, saturation coefficient and transferrin soluble receptor assays was performed in all patients included in the study.
Results |
Among the 46 patients included in the study, the prevalence of iron deficiency (defined as ferritinemia<100μg/L or ferritinemia between 100 and 299μg/L and a CST <20%) was 32.60%. None of the factors studied (age, sex, hypertension, diabetes, NYHA class, hemoglobin, CRP, LVEF) were statistically associated with iron deficiency.
Conclusion |
Iron deficiency is common in patients with impaired heart failure. However, in our daily practice, this comorbidity is not actively sought and treated hence the value of systematic screening to improve the symptomatology, functional capacity and quality of life of this category of patients.
