Purpose |
To assess the success rate of a matrix regenerating agent (RGTA) in the treatment of chronic corneal ulcers resistant to conventional treatments.
Methods |
Uncontrolled prospective observational study in patients with corneal neurotrophic ulcer (Stage 2 or 3 of the Mackie classification), unresponsive to standard medical or surgical treatments and managed with RGTA as an adjunctive treatment. Corneal ulcers were evaluated using slit-lamp examination and optical coherence tomography after 2 weeks, 1 month, 2 months and 3 months. Success was defined as complete corneal healing.
Results |
RGTA was administered to 20 patients (20 eyes) with chronic corneal ulcers of various causes, including keratoplasty (7 eyes, 35.0%), herpetic keratitis (5 eyes, 25.0%) and intracorneal ring (3 eyes, 15.0%). Total corneal healing was observed in 13/20 patients (65.0%) within 1 to 3 months. RGTA was discontinued due to partial healing/ulcer stagnation in 6 patients (30.0%), and aggravation of the ulcer in 1 patient (5.0%). Relapses were reported in 4 patients (20.0%) several months after cessation of RGTA treatment. No adverse reactions were noted.
Conclusions |
In the event of failure of conventional treatments, RGTA may be an alternative medical treatment for patients with chronic corneal ulcers, avoiding the need to resort to surgical treatment.

Objectif |
Évaluer le taux de succès d’un agent de régénération matriciel (RGTA) dans le traitement des ulcères cornéens chroniques résistants aux traitements standards.
Méthodes |
Étude observationnelle, prospective, non contrôlée chez des patients présentant un ulcère cornéen neurotrophique (stade 2 ou 3 selon la classification de Mackie), ne répondant pas aux traitements médicaux ou chirurgicaux standards et gérés par le RGTA en traitement d’appoint. Les ulcères cornéens ont été examinés à la lampe à fente et par tomographie de cohérence optique après 2 semaines, 1 mois, 2 mois et 3 mois. Le succès thérapeutique était défini par la guérison complète de l’ulcère.
Résultats |
Le RGTA a été administré à 20 patients (20 yeux) présentant des ulcères cornéens chroniques d’étiologies différentes, principalement des kératoplasties (7 yeux, 35,0 %), des kératites herpétiques (5 yeux, 25,0 %) et des anneaux intracornéens (3 yeux, 15,0 %). La guérison complète a été observée chez 13/20 (65,0 %) patients après 1 à 3 mois. Le RGTA a été arrêté après guérison partielle ou stagnation de l’ulcère chez 6 patients (30,0 %) et après aggravation de l’ulcère chez 1 patient (5,0 %). Une rechute a été rapportée chez 4 patients (20,0 %) plusieurs mois après l’arrêt du RGTA. Aucun effet indésirable dû au RGTA n’a été noté.
Conclusions |
En cas d’échec des traitements standards, le RGTA peut être une alternative médicale pour les patients présentant des ulcères cornéens chroniques, évitant le recours au traitement chirurgical.
