Résumé |
Le phénotype MSI des tumeurs coliques serait un marqueur de bon pronostic, avec un impact probablement plus marqué pour les stades II que pour les stades III. Cet avantage de survie pourrait être en rapport avec l’infiltration lympho-plasmocytaire observée dans les tumeurs MSI, expliquant ainsi l’existence d’une réaction immunitaire anti-tumorale plus efficace. Par ailleurs, le statut MSI serait également un biomarqueur capable de prédire l’absence d’efficacité de la chimiothérapie adjuvante par 5FU. En revanche, à l’instar des tumeurs MSS, les tumeurs MSI tireraient un bénéfice de survie à l’adjonction de l’oxaliplatine au 5FU adjuvant. Sur la base de ces données, le thésaurus national de cancérologie digestive propose pour les tumeurs MSI, une chimiothérapie adjuvante par fluoropyrimidine+oxaliplatine pour les stades III, et l’absence de traitement complémentaire pour les stades II (en dehors des tumeurs pT4b où la combinaison fluoropyrimidine+oxaliplatine peut être une option thérapeutique). Au-delà de ces recommandations, d’autres facteurs comme l’âge du patient et ses comorbidités doivent bien entendu être intégrés dans la discussion de traitement adjuvant des tumeurs MSI. La durée du traitement adjuvant (3 ou 6 mois) et le protocole utilisé (FOLFOX ou XELOX) doivent être basés sur les recommandations du consortium international IDEA en attendant les résultats des études translationnelles de cet essai. Enfin, les résultats très prometteurs de l’immunothérapie dans le cancer colorectal MSI métastatique ont permis le développement d’essais thérapeutiques évaluant les « immunes checkpoint blockers » en combinaison avec le FOLFOX dans le traitement du cancer du côlon MSI stade III.

The MSI phenotype in colon cancer is a good prognostic factor, with an impact probably more pronounced for stage II than stage III tumor. This survival advantage may be related to the tumor-infiltrating lymphocytes observed in MSI tumors, thus explaining the existence of a probably more effective anti-tumor immune response. In addition, the MSI status would also be a biomarker able to predict the lack of efficacy of adjuvant 5-fluorouracil (5FU) chemotherapy. In contrast, as observed in MSS colon cancer, the MSI tumors would have a survival benefit with the addition of oxaliplatin to adjuvant 5FU chemotherapy. Based on these data, the “French National Thesaurus of Digestive Oncology” suggests for patients with MSI colon cancer, an adjuvant chemotherapy combining fluoropyrimidine and oxaliplatin for stage III, and surgery alone without adjuvant chemotherapy for stage II (excepted for pT4b tumors in which the combination of fluoropyrimidine and oxaliplatin may be a therapeutic option). Beyond these recommendations, the discussion of adjuvant treatment in MSI tumors should also include other factors such as the patient's age and comorbidities. The duration of the adjuvant treatment (3 or 6 months) and the regimen used (FOLFOX or XELOX) should be based on the recommendations of the international IDEA consortium pending the results of the translational studies of this trial. Finally, the promising results of immunotherapy in metastatic MSI colorectal led to the development of clinical trials evaluating “immune checkpoint blockers” in combination with FOLFOX in the treatment of stage III MSI colon cancer.
