Abstract |
Purpose |
In the present study we constructed a new customized bite block and evaluated its effect on dosimetric and clinical features in radiotherapy.
Materials and methods |
The key point of this method is using the oral stent and silicon rubber through a procedure of making fine intraoral impressions. For type I customized bite block, only the upper and lower alveoli of the oral stent are coated with silicone rubber and impressions of the teeth can be obtained. For type II customized bite block, all parts of the oral stent are coated with silicone rubber to make sure impressions of the teeth, tongue and other intraoral tissues can be obtained. Sixty patients undergoing radiotherapy of nasal cavity, paranasal sinuses and oromaxillofacial area were evaluated. For each patient, intensity-modulated radiotherapy plans created with and without customized bite block were compared and data were collected on radiation-induced complications.
Results |
The total fabricating time was approximately 20minutes, and the cost was approximately 10 US dollars. Type I customized bite block is applicable to cancer of sinuses and nasal cavity. Type II customized bite block is applicable to cancer of the oral cavity, salivary glands, especially tongue cancer. The use of the customized bite block showed promising results in decreasing the radiation dose to organs at risk during intensity-modulated radiotherapy. No patients had oral mucositis (grade 3 or above), taste dysfunction, or xerostomia during and post radiotherapy.
Conclusion |
A customized bite block is easy-made and worthy of clinical use.

Objectifs |
Cette étude présente la méthode de la fabrication d’un embout buccal individualisé pour la radiothérapie du cancer de la tête et du cou et discute son influence sur la dosimétrieet sa valeur de son application clinique.
Matériels et méthodes |
Pour réaliser un embout buccal individualisé dans cette étude nous avons utilisé des empreintes endobuccales fines en silicone. Pour le type I, seules les alvéoles supérieure et inférieure de l’embout buccal buccale ont été revêtues de caoutchouc de silicone et des empreintes de dents ont pu être obtenues. Pour le type II, toutes les parties de l’embout buccal ont été recouvertes de silicone pour s’assurer que les empreintes des dents, de la langue et d’autres tissus intraoraux pouvaient être obtenues. Les résultats obtenus chez 60 patients irradiés dans la cavité nasale, les sinus paranasaux et la zone oromaxillofaciale ont été évalués. Pour chaque patient, les plans de radiothérapie conformationnelle avec modulation d’intensité créés avec et sans embout buccal ont été comparés et des données ont été collectées sur les complications radio-induites.
Résultat |
La fabrication d’un embout buccal individualisé prend environ 20minutes et le coût total est d’environ 10 dollars américains. Le type I est utilisé principalement pour le cancer des cavités nasosinusiennes, le type II pour les cancers de la cavité buccale et des glandes salivaires, en particulier pour le cancer de la langue. L’utilisation d’un embout buccal individualisé peut réduire la dose délivrée aux organes à risque. Au cours et après la radiothérapie, aucun patient n’a été atteint de mucite orale de grade 3 ou plus, de dysgueusie ou de xérostomie.
Conclusion |
La simplicité de la méthode de fabrication d’un embout buccal individualisé permet de lancer l’utilisation clinique.
