Résumé |
Environ 10 % des patients présentant un accident vasculaire cérébral sont traités par un anticoagulant oral, de type anti-vitamine K, antithrombine ou anti-Xa au moment de leur admission. Ces traitements vont modifier la stratégie thérapeutique usuelle à la fois des infarctus et des hémorragies intracérébrales. En présence d’un infarctus cérébral la décision de thrombolyse dépendra de la sévérité des symptômes, du délai d’admission depuis le début des symptômes, du délai depuis la dernière prise médicamenteuse et de dosages biologiques spécifiques. La thrombectomie sera priorisée devant toute occlusion proximale. Dans le cadre d’une hémorragie cérébrale l’utilisation de stratégies de réversions spécifiques du traitement anticoagulant reposant sur le CCP pour les AVK et les inhibiteurs du facteur Xa et l’idarucizumab pour le dabigatran, sera proposée en association au contrôle tensionnel. À l’issue d’une ischémie ou d’une hémorragie cérébrale l’indication et le délai de réintroduction du traitement anticoagulant seront évalués à la lumière du risque individuel thrombotique et hémorragique.

Approximately 10% of stroke patients are treated with oral anticoagulants, such as vitamin K antagonists antithrombin or factor Xa inhibitors, at the time of admission. These treatments will modify the usual therapeutic strategy for both infarction and intracerebral hemorrhage. In the presence of a cerebral infarction, the decision of thrombolysis will depend on the severity of the symptoms, the time of admission since the onset of symptoms, the delay since the last drug intake and specific assay results. Mechanical thrombectomy will be prioritized in patients with proximal occlusion. In the context of cerebral hemorrhage, the use of specific reversal strategies such as prothrombin complex concentrate for vitamin K antagonist and factor Xa inhibitors, and idarucizumab for dabigatran will be used in combination with the blood pressure management. Following ischemia or cerebral hemorrhage, the indication and time to reintroduce the anticoagulant treatment will be evaluated in light of the individual thrombotic and hemorrhagic risk.
