Résumé |
L’évaluation visuelle de l’ictère n’est pas toujours aisée, surtout chez le nouveau-né à peau mate ou noire. Le dépistage instrumental par bilirubinomètre transcutané est le seul moyen non invasif de dépistage en maternité. Ce cas clinique permet de souligner l’intérêt du dépistage précoce de l’ictère en maternité surtout chez le nouveau-né de race noire.
Observation clinique |
Il s’agit d’un nouveau-né à terme, de sexe masculin, hospitalisé en néonatologie après 48 heures de vie pour ictère sévère. L’interrogatoire révélait déjà un ictère la veille, quantifié par bilirubinomètre transcutané, mais non pris en compte par la soignante, estimant cette mesure non fiable chez le nouveau-né de race noire. Le groupe sanguin de la mère était O positif, celui du père A positif. Le test de Coombs direct est positif avec présence d’anticorps anti-A au test d’élution. La bilirubine sanguine lors de la prise en charge était alors supérieure au seuil d’exsanguino-transfusion. Le traitement a consisté en une photothérapie intensive et d’une exsanguino-transfusion. L’IRM cérébrale réalisée à 8 jours de vie ne montre pas d’anomalies au niveau des noyaux gris centraux.
Conclusion |
Le dépistage de l’ictère par bilirubinométre transcutanée a toute sa place en maternité y compris chez l’enfant à peau noire. Par ailleurs, le dosage sanguin doit être systématique chez tout nouveau-né lorsque les valeurs mesurées avec le bilirubinomètre avoisinent le seuil de photothérapie.

The visual evaluation of jaundice is not always easy, especially in the newborn with matte or black skin. Transcutaneous bilirubinometer screening is the only non-invasive means of screening in the maternity ward. This clinical case highlights the value of early detection of jaundice in maternity especially in the newborn black race.
Clinical observation |
This is a term newborn, male, hospitalized in neonatology after 48hours of life for severe jaundice. The interrogation already revealed jaundice the day before, quantified by transcutaneous bilirubinometer, but not taken into account by the caregiver, estimating this measure unreliable in the black newborn. The blood group of the mother was O positive, that of the father A positive. The direct Coombs test is positive with the presence of anti-A antibodies in the elution test. The blood bilirubin at the time of the management was then higher than the threshold of exsanguino-transfusion. The treatment consisted of intensive phototherapy and exsanguino-transfusion. Cerebral MRI performed at 8 days of life does not show abnormalities in the basal ganglia.
Conclusion |
The detection of jaundice by transcutaneous bilirubinometer has its place in maternity including in the child with black skin. In addition, the blood test should be systematic for all newborns when the values measured with the bilirubinometer are close to the phototherapy threshold.
