La préparation préopératoire du patient avant chirurgie majeure peut être optimisée par une approche multimodale, telle que la « préhabilitation ». Elle consiste en un programme d’une durée de 4 semaines, comprenant une prise en charge médicale (amélioration des comorbidités), une préparation physique (exercices personnalisés en résistance et en endurance), une préparation nutritionnelle (apport d’au moins 1g/kg/j d’apport de protéines) et un soutien psychologique (prise en charge de l’anxiété). Ces divers renforcements permettent d’amener le patient à l’intervention dans de meilleures conditions. La mise en œuvre d’un programme de préhabilitation nécessite toutefois de repenser le parcours préopératoire du patient : « identification » au plus tôt des patients éligibles, organisation de l’ensemble des consultations initiales et du suivi, maintien d’un lien avec l’hôpital pendant le programme, personnalisation du projet thérapeutique. Notre établissement est passé en 18 mois d’une phase pilote à une proposition en routine pour certains actes. Cette expérience et les éléments mis en place nous semblent intéressants à partager car ce programme de préhabilitation précède logiquement la réhabilitation rapide postopératoire ayant pour objectif final une diminution des complications et un retour au domicile dans de meilleures conditions physiques.

The preoperative optimisation of a patient planned for major surgery can be improved by a multimodal approach named “Prehabilitation”. It consists of a program lasting about 4weeks and includes medical care (to stabilise or improve active comorbidities), physical preparation (with personalised resistance and endurance exercises), nutritional preparation (intake of at least 1g/kg/day of protein) and psychological support (anxiety management). The aim of this multimodal preparation is to bring the patient to the operation in a better condition. The implementation of a prehabilitation program requires to think about the patient's preoperative pathway: “identification” as soon as possible of eligible patients, organisation of every initial visit and follow-up, maintenance of a link with the hospital during the program, personalisation of the therapeutic project. During an 18-month period, our institution has developed a routine care with prehabilitation for some surgical repair. This experience seems interesting to share with other care-providers because this prehabilitation program is logically followed in the postoperative period by enhanced recovery with the ultimate objective of reducing postoperative complications and returning home in better physical conditions.
