Tolerance of hypofractionated stereotactic radiotherapy for hepatic tumours

Objectif de l’étude |
Il s’agissait d’un étude monocentrique rétrospective de la tolérance de la radiothérapie stéréotaxique hypofractionnée hépatique.
Matériels et méthode |
De décembre 2013 à juin 2016, les dossiers de 25 patients ont été évalués après radiothérapie stéréotaxique hypofractionnée sur accélérateur Novalis TX® de 42 à 60Gy en trois à cinq fractions. Les critères modified Response Evaluation Criteria in Solid Tumours (mRECIST) et RECIST ont été utilisés pour l’évaluation du contrôle local et la classification Common Terminology Criteria for Adverse Events, version 4.0 pour l’évaluation de la toxicité.
Résultats |
Le suivi médian était de 15,3 mois. La tolérance au traitement a été satisfaisante avec aucun évènement indésirable aigu de grade 3 ou plus et un seul tardif de grade 3 ou plus. Il a été mis en évidence un risque d’hémorragie tardive de l’artère hépatique plus élevé chez les deux patients porteurs d’une prothèse biliaire au contact de l’artère (p=0,006) et dans le volume irradié, sans corrélation avec la dose délivrée aux gros vaisseaux.
Conclusion |
La radiothérapie stéréotaxique hypofractionnée hépatique est un traitement efficace en termes de contrôle local avec peu d’effets secondaires à court et à long termes. Il convient d’être prudent chez les patients porteurs de prothèse biliaire se trouvant à proximité du volume cible car il peut exister un risque d’hémorragie de l’artère hépatique au contact de la prothèse.

Purpose |
The purpose of the study was to evaluate the outcomes of stereotactic radiation therapy for primary and secondary liver tumours in Jean-Perrin cancer centre (Clermont-Ferrand, France) in terms of efficacy and safety.
Materials and methods |
Between December 2013 and June 2016, 25 patients were included. Treatment was performed on a linear accelerator Novalis TX®. The prescription dose was 42 to 60Gy in three to five fractions. Local control at 1 year was evaluated with modified Response Evaluation Criteria in Solid Tumours (mRECIST) and RECIST criteria. Acute and late toxicity were evaluated with Common Terminology Criteria for Adverse Events (CTCAE) v4.0 criteria.
Results |
Median follow-up was 10.5 months. Treatment tolerance was good with few side effects grade 3 or above, no acute toxicity and only one late toxicity. We have highlighted that hepatic artery haemorrhage was associated with the presence of a biliary prosthesis in contact with the artery (P=0.006) and in the irradiation field. There was no correlation with the dose delivered to the artery and hepatic artery haemorrhage.
Conclusion |
Stereotactic radiation therapy for liver tumours allows a good local control with few secondary effects. Caution should be exercised when treating patients with biliary prostheses in the vicinity of the target volume because there is a risk of haemorrhage of the hepatic artery in contact with the prosthesis.
