Bleeding events following initiation of anticoagulation in a geriatric population with atrial fibrillation

Contexte |
Les anticoagulants oraux directs (AOD) sont recommandés en première intention devant les antivitamines K (AVK) dans la prévention des évènements thromboemboliques liés à la fibrillation atriale non valvulaire (FANV). Leur risque hémorragique reste peu étudié dans la population gériatrique en « vraie vie ».
Objectif |
Comparer la fréquence des hémorragies graves sous AVK et AOD dans une population gériatrique nouvellement traitée pour FANV.
Méthode |
Nous avons réalisé une étude observationnelle rétrospective de type cohorte historique avec suivi aux dernières nouvelles, monocentrique. Nous avons inclus des patients hospitalisés en SSR gériatrique, nouvellement anticoagulés pour FANV, entre le 01/01/2014 et le 31/12/2015. Le suivi a été réalisé par appel téléphonique aux médecins traitants début 2017, soit 3 ans après les premières inclusions.
Résultats |
Quatre-vingt onze patients ont été inclus, 62 sous AVK et 29 sous AOD. La durée moyenne de suivi était de 18 mois. L’âge moyen de notre population était de 86,6 ans, l’indice de Charlson ajusté à l’âge moyen était de 5,6 points, le MMSE moyen à 21/30. Soixante-huit pour cent des patients étaient chuteurs. Le poids était significativement plus faible dans le groupe AVK que dans le groupe AOD (60kg vs 66,5kg), avec une plus grande proportion de patients avec IMC<21kg/m2 (24 % vs 7 %). Les autres caractéristiques des patients, entre les groupes AVK et AOD, n’étaient pas différentes. Nous avons constaté 5 hémorragies graves, toutes dans le groupe AVK, sans différence significative avec le groupe AOD. La survie sans hémorragie grave était plus faible dans le groupe AVK (log-rank p=0,003), en lien avec un plus grand nombre de décès (21 vs 2).
Conclusion |
Les hémorragies graves sous anticoagulants restent rares dans une population gériatrique anticoagulée pour FANV. La surmortalité observée ici dans le groupe AVK reflète probablement leur prescription chez des patients plus à risques.

Background |
Direct Oral Anticoagulants (DOAC) are preferred over Vitamin K Antagonists (VKA) for stroke prevention in patients with Non-Valvular Atrial Fibrillation (NVAF). Their bleeding risk remains poorly studied in real-life geriatric care.
Objective |
To compare the incidence of major bleeding under VKA and DOAC in a elderly population newly treated for NVAF.
Methods |
We conducted a retrospective monocentric observational study. Inpatients with recent instatement of anticoagulants for NVAF were included from two geriatric rehabilitation wards between 01/01/2014 and 31/12/2015. Follow-up data was obtained by phone calls to their general practitioner in the beginning of 2017, 3 years after the first inclusions.
Results |
Out of ninety one patients included, 62 and 29 patients initiated VKA and DOAC respectively. The mean follow-up was 18 months. Mean age was 86.6 years, the mean age-ajusted Charlson index score was 5.6 points, and the mean MMSE was 21 points. Sixty eight per cent had experienced falls. Weight was significantly lower in the VKA group (60 vs 66.5kg), with a larger proportion of patients with BMI <21kg/m2 (24 vs 7%). Other characteristics were similar between groups. We observed 5 major bleedings, all in the VKA group, with no significant difference compared to the DOAC group. Survival without major bleeding was lower in the AVK group (log-rank P=0.003), linked to a larger number of death (21 vs 2).
Conclusion |
Major bleeding with oral anticoagulants is rare in a geriatric population receiving anticoagulants for NVAF. The higher mortality in the VKA group in this study probably reflects their prescription to patients with greater risks.
