Toute anesthésie peut se compliquer d’arrêt cardiaque, événement rare mais dont l’incidence est plus élevée sous anesthésie générale. Les circonstances de survenue (après l’induction, l’intubation, durant la chirurgie) orientent le diagnostic étiologique. Le diagnostic est immédiat au bloc opératoire du fait du monitorage. Le traitement n’est pas spécifique sauf pour certaines étiologies (intoxication par les anesthésiques locaux, hyperthermie maligne). Dans les suites, il faut poursuivre l’assistance respiratoire et l’anesthésie jusqu’à l’évaluation des lésions neurologiques par l’imagerie cérébrale. La prise en charge notamment des lésions neurologiques, ne diffère pas de celle d’un arrêt cardiaque anoxique d’autre cause. L’hypothermie thérapeutique n’a pas fait la preuve de son efficacité. Du fait de la précocité de la réanimation, le pronostic est meilleur que celui des arrêts cardiaques extra hospitaliers.

Cardiac arrest may occur, although very rarely, during each anaesthetic process. The timing (after induction, intubation or during surgery) is indicative of the mechanism. The diagnosis is performed immediately thanks to the monitoring. The management is not specific except for the treatment of the mechanism (local anaesthetic intoxication, malignant hyperthermia). After resuscitation, ventilator support and anaesthesia have to be maintained until assessment of the neurological status. The management of neurologic consequences is not different to the one of any anoxic brain damage. Controlled hypothermia has not been demonstrated to be effective in this setting. Thanks to the immediate diagnostic and management, the prognostic is better than the one of out of hospital cardiac arrest.
