Evaluation of maintained physical capacity one year after cardiac rehabilitation (EMAP study) in coronary patients - 06/01/20

Introduction |
There are few data about the follow-up in coronary patients after cardiac rehabilitation (CR). Do they maintain a regular physical activity? Do they improve or decrease their cardiorespiratory fitness (CRF)?
Aims |
To measure the CRF in coronary patients during CR and one year later, in a French multicentre registry.
Methods |
Patients were included after an acute coronary syndrom (77%) and/or revascularization less than 3months ago. They underwent a CR programme with 2exercise testing (ET) on a bicycle at the beginning (ET1), the end of CR (ET2) and at one-year (ET3), with the same protocol and with medical treatment.
Results |
Two hundred and fifty nine patients were included (mean age=60±10y, males=88.8%) in 16French CR centres. Left ventricular ejection fraction was 55.3±9, coronary stenosis were single-vessel (47.1%), two-vessels (25.8%), three-vessels (27%). Revascularization was complete (81.5%), incomplete (16.6%), absent (2%). The maximal workloads were 110±37 (ET1), 139±43 (ET2) and 144±46Watts (ET3) (P<0.001). The estimated METs were respectively 5.3±1.4, 6.4±1.6 (P<0.001) and 6.6±1.7 (P<0.002). One hundred and nineteen patients (46%) improved their CRF (ET3>ET2), 78 (30%) maintained (ET3=ET2), 34 (13%) decreased (ET1<ET3<ET2) and 28 (10.8%) lost the benefit of CR (ET3≤ET1). The results were similar with age and sex, but lower in current smokers (13.5%) and in diabetic patients (18.1%).
Conclusion |
Contrary to the common belief in cardiological circles, coronary patients seem to maintain or even improve their CRF one year after a CR programme.
