Le dépistage du cancer du col par un frottis tous les trois ans pour les femmes de 25 à 65 ans existe depuis les années quatre-vingt-dix. La cytologie en milieu liquide a progressivement remplacé la cytologie conventionnelle car cette technique améliore la qualité du prélèvement. La lecture au microscope est facilitée par la sélection de champs sélectionnés par un système de prélecture automatisée. Grâce au développement de techniques d’analyse d’images, la digitalisation est maintenant possible permettant une lecture à distance des frottis. Le double marquage de la p16/ki67 en immunocytochimie permet de détecter une infection transformante par un HPV à haut risque. La performance de cette approche a montré une bonne sensibilité et spécificité pour le diagnostic d’une lésion histologique de type CIN 2+ sur des frottis ASC-US et LSIL, en particulier chez les femmes jeunes. La Haute Autorité de santé a recommandé, en juillet 2019, le test HPV seul en dépistage primaire après 30 ans car il est plus sensible que la cytologie pour le diagnostic d’un CIN2+ et plus efficace pour prévenir les cancers invasifs. Un test HPV positif sera suivi sur le même prélèvement d’un examen cytologique. La mise en place d’un programme organisé national combinée à l’introduction du test HPV en dépistage primaire après 30 ans et à la vaccination des adolescents devrait permettre de diminuer l’incidence et la mortalité des cancers invasifs du col utérin dans un avenir proche en France.

Cervical screening recommendation to perform a Pap smear every three years for women 25 to 65 exists since 1990. The liquid based cytology (LBC) has replaced the conventional smear because the quality of the sample was proven better. LBC used manually make reading under the microscope easier by reading selected fields with cameras connected to computer software. With advanced image analysis, the digitalisation of LBC is now feasible and allows remote review. The simultaneous detection of p16 and Ki-67 expression within the same cervical epithelial cell called p16/ki67 dual stained cytology corresponds to a morphology-independent marker of an HPV transformation infection. The clinical performance of this approach in cases categorized as ASC-US or LSIL indicate that p16/Ki67 dual-stained cytology provides a high sensitivity and a high specificity for detecting underlying CIN2+, especially in young women. The Haute Autorité de santé has recommended in July 2019 the HPV primary screening alone after 30 years because it is more sensitive for the detection of CIN 2+ compared to cytology and is more effective in preventing invasive cervical cancer. The triaging of the positive HPV test will be performed with cytology in the residual material. National organized screening program with the introduction of the HPV primary test after 30 years combined with vaccination of the teenagers should improve the incidence and the mortality of the invasive cancer of the cervix in the near future in France.
