Importance du complexe ligamentaire postéromédial trapézométacarpien - 01/01/01

La pronosupination du pouce a pour l'homme une très grosse importance à la fois pratique et symbolique. La supination du pouce est l'apanage de la main qui caresse. Sa pronation est celui de la main qui saisit. Les surfaces articulaires du trapèze et du premier métacarpien ne sont pas suffisemment congruentes pour assurer efficacement la pronosupination du pouce, mais leur configuration concave-convexe (en selle) détermine deux axes de rotation «fondamentaux» distincts et orthogonaux, séparés l'un de l'autre, et deux plans de rotations «privilégiés» correspondant l'un aux mouvements de la pince pollicidigitale latérale, l'autre à ceux de l'opposition et de la contre opposition. Les ligaments se situent de part et d'autre de l'articulation trapézométacarpienne. Le complexe ligamentaire postéromédial se compose du ligament oblique postérieur, du ligament oblique antérieur, du ligament intermétacarpien et d'un quatrième ligament, le plus antéromédial, que nous avons appelé «rétinaculométacarpien» en raison de ses insertions proximales. Son rôle de suspension et d'ancrage du tubercule postéromédial de la base du premier métacarpien est essentiel. La suspension assure la stabilité de ce dernier. L'ancrage lui imprime deux types de mouvements: 1) un mouvement discret de translation le long de la concavité articulaire trapèzienne lors de sa rotation dans le plan de la pince pollicidigitale latérale; 2) un mouvement beaucoup plus important, caractérisé par une pronosupination de 90 degrés de celui-ci lors lors de sa rotation dans le plan de l'opposition. Le ligament antérolatéral se borne à renforcer l'action des ligaments postéromédiaux dans les positions extrêmes lors de la fermeture de la pince pollicidigitale latérale et lors de l'opposition du pouce. En cas de subluxation trapézométacarpienne, il s'oppose à la luxation complète. Ces diverses constatations ont d'importantes répercussions cliniques et thérapeutiques.

Abstract |
Pronation and supination of the human thumb has both a practical and a symbolic importance. In supination the thumb caresses but in pronation it grasps. The articular surfaces of the trapezium and first metacarpal are not congruent enough in themselves to assure pronation and supination, but their reciprocal saddle shape causes them to have two distinct orthogonal “fundamental” axes of rotation, and hence two “privileged” planes of rotation. The one corresponds to the movements of the lateral pinch grip and the other to opposition and counter-opposition of the thumb. The important ligaments are situated on both sides of the trapeziometacarpal joint. The posteromedial ligamentous complex is composed of the posterior oblique ligament, the anterior oblique ligament, the intermetacarpal ligament, and a fourth or more anteromedial ligament which we have called the retinaculometacarpal ligament by reason of its proximal insertion. It plays an essential role in suspending and anchoring the base of the first metacarpal. By its action it induces two types of motion: 1) a discrete translation of the metacarpal along the articular concavity of the trapezium in lateral pinch grip; 2) a more important motion characterised by a prosupination of 90° during its rotation in the plane of opposition. The anterolateral ligament's action is restricted to strengthening the action of the posteromedial ligaments in the extreme positions of closing of lateral pinch and thumb opposition. In cases of trapeziometacarpal subluxation it prevents a complete dislocation. These various observations have significant clinical and therapeutic implications.
