Résumé |
Nous rapportons les résultats à long terme de la première prothèse totale trapézométacarpienne dans une série personelle de 13 cas avec un recul de 12 à 17 ans. Cette prothèse de type «rotule» avec un couple de frottement métal-polyéthylène est cimentée. Plusieurs modifications ont été apportées dans le temps comme l'augmentation de l'arc de mobilité, une meilleure adaptation à la forme du canal médullaire, l'adjonction d'une pièce intermédiaire qui, avec une pince gabarit spéciale permet un rétablissement de la longueur du premier rayon. L'état de l'articulation métacarpophalangienne est très important : il existe chez les deux tiers des individus normaux une possibilité d'hyperextension métacarpophalangienne d'environ 20°. Quand un descellement métacarpien se produit, le premier rayon devient plus court et une hyperextension métacarpophalangienne peut se révéler. Celle-ci induit une déviation en adduction du premier métacarpien, elle-même à l'origine de forces de cisaillement qui produisent un descellement de la cupule (descellement séquentiel). Le maintien ou le rétablissement de la longueur du premier rayon est le principe majeur de la reconstruction prothétique trapézométacarpienne. Par ailleurs, une arthrodèse métacarpophalangienne est une contre-indication à la prothèse trapézométacarpienne. Cette prothèse ne doit être indiquée que comme dernière ressource en cas de douleur sévère. Elle est contre-indiquée en cas d'utilisation intensive sur une main dominante. Le trapèze doit être assez profond pour admettre la cupule. L'auteur recommande un voie postérieure respectant les ligaments antérieurs,facilitant la préparation des extémitées et le bon centrage de la cupule.

Abstract |
We report the long term outcome of the trapeziometacarpal prosthesis in a personal series of 13 cases with a follow-up ranging from 12 to 17 years. This ball and socket prosthesis with a metal on polyethylen pairing is cemented. Several modications were performed along the years: such as increased range of motion, better fit betwen the metacarpal medullary canal and the proximal part of the stem and addition of a versatile, intermediate component in order to re-establish length of the first ray. The status of the metacarpo-phalangeal joint is critical. Natural hyperextension of this joint has been seen in about two-third of th normal individuals. If the metacarpal component become loose, the thumb column become shorter and hyperextension of the metacarpophalangeal joint is followed by an adduction contracture of the metacarpal which produces a shear force component on the cup and finally the loosening of the cup (sequential loosening). Keeping or re-establishing the length of the first ray is the major principle in the trapeziometacarpal prosthesis. On the other hand, metacarpophalangeal arthrodesis is a contraindication for a trapeziometacarpal prosthesis. Trapeziometacarpal prosthesis should only be used as a last resort when there is severe pain. If the dominant hand is involved, intensive use is a contraindication. The trapezium must be large enough to accomodate the cup. The author recommends a posterior approach which preserve the anterior ligaments and allows a proper bone resection and a good positionning of the cup.
