Les problèmes de l'articulation métacarpophalangienne dans le traitement chirurgical de la rhizarthrose par prothèse contrainte type ARPE - 01/01/01

Résumé |
L'ankylose progressive de l'articulation trapézométacarpienne en flexion-adduction, avec fermeture de la première commisure, dans la rhizarthrose évoluée entrame peu à peu une dislocation compensatrice de l'articulation métacarpophalangienne en hyperextension dans le plan sagittal et en abduction dans le plan frontal. Cette déformation de la MP réductible au début puis irréductible, aboutit au classique pouce en Z dans le plan sagittal. Une déformation en Z moins connue peut se voir également dans le plan ftontal par distension du LLI. Lors du traitement chirurgical de la rhizarthrose par arthroplastie il convient de corriger cette déformation secondaire de la MP pour obtenir un résultat optimum : la correction du classique pouce en Z sagittal est facilement obtenue tant que cette déformation est réductible par «ouverture» du 1er métacarpien en remettant en tension le long abducteur du pouce. Lorsqu'elle est irréductible cette déformation ne peut être traitée que par une arthrodèse MP qui contre-indique alors la mise en place d'une prothèse TM de ARPE®. La correction du pouce en Z dans le plan frontal nécessite de réparer le LLI de la MP par ligamentorraphie (retension) ou ligamentoplastie.

Abstract |
Progressive ankylosis of the trapeziometacarpal joint in flexion-adduction with closure of the first web in advanced trapeziometacarpal osteoarthritis gradually leads to compensatory dislocation of the metacarpophalangeal joint with hyperextension in the sagittal plane and abduction in the frontal plane. This deformity of the MP joint, initially reducible, but subsequently irreducible, results in the classical Z' deformity of the thumb in the sagittal plane. A less well known Z' deformity can also occur in the frontal plane due to distension of the medial collateral ligament. Surgical treatment of trapeziometacarpal osteoarthritis by arthroplasty must correct this secondary deformity of the MP joint to obtain an optimal result. The classical sagittal Z' deformity of the thumb can be easily corrected while this deformity is still reductible by releasing the fist metacarpal by tightening the abductor pollicis longus. When it is irreducible, this deformity can only be treated by MP arthrodesis, which contraindicates insertion of the ARPE trapeziometacarpal implant. Correction of the frontal Z' deformity of the thumb requires repair of the medial collateral ligament of the MP joint by ligamentorraphy (retightening) or ligamentoplasty
