Résumé |
Introduction |
Cette étude visait à dresser un bilan de la conciliation médicamenteuse (CM) déployée dans l’unité d’hébergement de courte durée (UHCD) de 32 lits afin d’en optimiser l’organisation.
Matériel et méthodes |
Un recueil a été réalisé sur deux mois : priorisation des patients, réalisation ou non d’une CM, interventions pharmaceutiques (IP) réalisées et parcours des patients à leur sortie de l’UHCD.
Résultats |
Un total de 59 % des CM jugées prioritaires ont été réalisées, soit 138 CM effectuées (28 % des patients, n=489). Un total de 45 % des patients ont été transférés vers un service ne bénéficiant pas de CM. 78 % des conciliations ont révélé au moins une divergence non intentionnelle donnant lieu à des IP avec un taux d’acceptation de 83 %.
Discussion |
Les critères de priorisation retenus dépendent de l’âge du patient, de son traitement chronique, de sa provenance et de la durée prévisionnelle du séjour.
Conclusion |
La méthode de priorisation des patients a été mise à jour. Une réorganisation de l’activité ainsi que la formation de nouveau personnel à la conciliation ont été depuis réalisées.

Introduction |
The aim of this study was to review the medication reconciliation (MR) in the 32-bed Emergency Department (ED) to improve this activity organization.
Research design and methods |
Following data were collected during 2 months: patient prioritization, MR done or not, pharmaceutical interventions (PI) recorded and patient path after leaving the ED.
Results |
A total of 59% of MR considered as a priority were acheived, with a total of 138 MR realized (28% of the patients, n=489). A total of 45% of the patients were transferred to a care unit where patients are not reconciled. For 78% of reconciled patients, at least one unintentional discrepancy was found, followed by PI with an acceptance rate of 83%.
Discussion |
Criteria defined for prioritizing patient for MR are: patient age, provenance, chronic treatment and expected duration of hospital stay.
Conclusion |
The approach defined for prioritizing patient for MR has been updated. The reorganization of the reconciliation activity and the training of new qualified staff are now done.
