Abstract |
Background and objectives |
More and more surgeons are changing their surgical procedures by using mini-invasive technology with the aim of reducing morbidity. Robotized technology for the surgery of narrow spinal space, for the resection of intervertebral foramen tumor, is a valuable option that should be considered. The authors report the case of a patient who underwent robotic surgery for the treatment of a paravertebral schwannoma and show the benefits of this approach.
Case report |
A 53-year-old man was treated, in 1995, via an open right latero-thoracic approach for a T8–T9 thoracic schwannoma with a complete resection. Twenty years later, the patient complained of recurrent T8–T9 thoracic pain with unilateral radicular irradiation related to a recurrent schwannoma with a right foraminal extension. Using robotic technology (Da Vinci®), a complete resection was achieved and confirmed by a postoperative MRI. The patient, who was free of neurological symptoms, discharged the hospital at day 2.
Comments |
Robot-assisted surgery by Da Vinci® robot is a very useful approach for the treatment of spinal paravertebral schwannoma. Thanks to multi-directional arms and 3D vision, this technology is safe and effective about quality of resection. The use of mini-invasive technology should be routinely discussed for lesions in that specific location.

Dans de nombreux domaines et c’est le cas en chirurgie thoracique, l’essor des outils robotisés offre de nouvelles perspectives. Ainsi, la chirurgie des espaces confinés rachidiens, telle que l’exérèse de lésion thoracique pénétrant le foramen intervertébral, reste un challenge chirurgical dont il faut réduire la morbidité. Nous présentons le cas d’un patient opéré d’une récidive de schwannome paravertébral en chirurgie mini-invasive robotisée.
Observation |
Un patient âgé de 53 ans a été traité en 1995 d’un schwannome thoracique de niveau T8–T9 par une approche latéro-thoracique classique. L’exérèse a été jugée macroscopiquement complète. Vingt ans plus tard, le patient rapportait la réapparition de douleurs thoracique avec une irradiation en hémi-ceinture de niveau T8–T9. L’IRM retrouvait une récidive lésionnelle avec une extension au foramen T8–T9 droit. Une chirurgie robot-assistée (robotDa Vinci®) permettait de réaliser une exérèse complète confirmée par l’imagerie postopératoire. Le patient sortait à j2 de l’hôpital sans complication.
Commentaires |
La chirurgie assistée par le robot Da Vinci® propose une approche convaincante de l’exérèse des schwannomes paravertébraux grâce à l’utilisation d’instruments fins, articulés et robotisés ainsi qu’une vision 3D. Ces nouvelles procédures, qui sont peu morbides et n’altèrent en rien la qualité de l’exérèse chirurgicale, doivent être systématiquement discutées et mises en balance avec les techniques chirurgicales plus classiques.
