Résumé |
Introduction |
Un audit réalisé en 2014 a montré une divergence lors de la cotation des interventions pharmaceutiques (IP) au sein de notre établissement.
Objectif |
Réaliser un outil d’aide à la cotation et d’évaluer son efficacité sur l’homogénéité des cotations.
Matériel et méthode |
Il s’agit d’une étude prospective multicentrique. Des logigrammes sont créés et intègrent des items de la Société française de pharmacie clinique et des problématiques de logiciovigilance. La cotation de l’IP s’effectue en partant du problème soulevé. Cent IP extraites aléatoirement de la vie réelle ont été cotées « avant » et « après » mise en place des logigrammes, par 23 intervenants à l’analyse pharmaceutique. Le test du Kappa (κ) de Fleiss détermine les taux de concordance entre les participants pour chaque phase.
Résultats |
Seize logigrammes regroupés en quatre thèmes ont été créés et validés en staff pharmaceutique. Le taux de concordance globale augmente entre les deux phases (κ « avant »=0,625 et κ « après »=0,709).
Discussion et conclusion |
La création de cet outil permet d’augmenter l’homogénéité de la cotation des IP. Les logigrammes sont intégrés au système qualité et font partis de la formation initiale à l’analyse pharmaceutique.

Introduction |
An audit carried out in 2014 showed a discrepancy in the rating of pharmaceutical interventions (PI) within our institution.
Objective |
To develop a tool and evaluate its effectiveness on the homogeneity of the ratings.
Material and methods |
This is a prospective multicentre study. Flowcharts are created and integrate items from the French Society of Clinical Pharmacy and prescription software vigilance issues. The PI rating is based on the problem identified. One hundred PIs randomly extracted from real life were rated “before” and “after” the implementation of flowcharts. The Kappa test of Fleiss (κ) determines the concordance between participants for each period.
Results |
Sixteen flowcharts grouped into four themes were created and validated by pharmaceutical staff. The overall concordance rate increases between the two periods (κ “before”=0.625 and κ “after”=0.709).
Discussion and conclusion |
The creation of this tool increases the homogeneity of PI coding. Flowcharts are integrated into the quality system and are part of the initial training in pharmaceutical analysis.
