Objectif |
Évaluer les résultats obtenus après une rééducation accélérée en chaîne cinétique fermée chez des patients ayant bénéficié d’une reconstruction du ligament croisé antérieur par transplant libre au tendon rotulien, sous arthroscopie, selon la technique de Kenneth Jones.
Matériel et méthode |
Étude prospective, descriptive, continue sur 100 patients dont 96 pratiquaient une activité sportive régulière avant l’intervention. Ces patients ont été pris en charge au sein du service MPR du CHU Mustapha Pacha d’Alger durant 6 ans, selon un programme de rééducation basé sur le travail en chaîne cinétique fermée qui tient compte du processus de ligamentisation (durée estimée chez l’homme à 3 ans) et des contraintes mécaniques sur le genou et sur la plastie. L’évaluation a été effectuée à j0 et j21 et à 2, 4 et 6 mois postopératoires et a concerné la douleur, l’épanchement intra-articulaire, la mobilité articulaire, la force musculaire et l’amyotrophie. La stabilité a été évaluée à 6 mois postopératoires en utilisant les scores IKDC, ARPEGE et Lysholm. Le taux de reprise de l’activité sportive a été évalué à 6 mois postopératoires.
Résultats |
Douleur minime : 2,99/10 sur l’échelle EVA. Épanchement intra-articulaire abondant à j21 chez 50 % des patients ; absent à 6 mois chez 78 % des cas. Mobilité progressivement améliorée. Force musculaire identique au côté controlatéral à 6 mois postopératoires pour le quadriceps chez 65 % des sujets et pour les ischiojambiers chez 70 % des cas. Amyotrophie minime de 1cm par rapport au côté controlatéral, soit une perte de 10 % de la masse musculaire. Laxité résiduelle minime sans signe d’instabilité chez 9 % des patients. Résultats satisfaisants après une rééducation en chaîne cinétique fermée : score IKDC objectifs A et B (normal et presque normal) : 80 % ; score IKDC subjectif : 84,25 points sur 100 ; score Lysholm : 93,10 points sur 100 ; score ARPEGE excellent et bon : 94 % ; reprise de l’activité sportive pour 52,1 % des sujets.
Discussion |
Malgré les bons résultats cliniques et fonctionnels obtenus, le taux de reprise de l’activité sportive est resté modeste à cause de facteurs extrinsèques d’ordre social, professionnel ou psychologique, entre autres; l’appréhension est également un facteur fréquent. Elle est liée à un dysfonctionnement minime, non pas du quadriceps mais des ischiojambiers. D’où l’intérêt de la rééducation proprioceptive des ischiojambiers.
Conclusion |
Le protocole de rééducation en chaîne cinétique fermée, apparu dans les années 1980, a grandement démontré son utilité. Il est aujourd’hui appliqué dans la prise en charge des ligamentoplasties du genou.

Objective |
Evaluate the results following an accelerated rehabilitation using closed kinetic chain on patients who had previously received a reconstructive therapy of the anterior cruciate ligament via transplantation of the patellar tendon under arthroscopy based on the Kenneth Jones technic.
Material and method |
This prospective and descriptive study was conducted over 100 patients, 96 of whom were involved in regular sports activity. They were all taken care of by the rehabilitation unit of CHU Mustapha Pacha of Algiers during 6 years. They all received the same medical rehabilitation program based on the closed kinetic chain, that considered the ligamentization phenomenon (estimated duration in men: 3 years) and mechanical constraints imposed to the knee and the plasty. The evaluation took place at day 0, day 21, month 2, month 3 and month 6 post-surgery, focusing on 5 major elements: pain, intra-articular effusion, joint mobility, muscle strength and amyotrophy. Stability of the knee was evaluated at month 6 post-surgery using IKDC, ARPEGE and Lysholm scores. The rate of activity recovery was evaluated at month 6 post-surgery.
Results |
Minimal pain: 2.99/10 on EVA scale. Profuse intra-articular effusion at day 21 in 50% of patients; absent at month 6 in 78% patients. Mobility continuously improved. Muscular strength is identical to the contra-lateral side at month 6 post-surgery for 65% patients in regards to the quadriceps muscle and for 70% patients in regards of the hamstring muscle. A 1cm amyotrophy compared to the contralateral side, with a 10% loss of the muscular mass. Minimal residual laxity without any signs of instability for 9% of patients. Satisfactory results post closed kinetic chain rehabilitation: IKDC score objective A and B (normal to almost normal): 80%; IKDC score subjective: 84.24 points out of 100; Lysholm score: 93.10 point over 100; ARPEGE score: extremely good 94%; return to physical activity for 52.1% subjects.
Discussion |
Despite the satisfactory clinical and functional results obtained, the level of physical activity recovery remains moderate due to external influences such social, professional and psychological components; anxiety plays a major role as well. The recovery is finally more linked to a minimal dysfunction of the hamstring muscles rather than the quadriceps. That is why the proprioceptive rehabilitation of the Hamstring is a fundamental step in the recovery process.
Conclusion |
The rehabilitation protocol in closed kinetic chain developed in the eighties shows its importance up to these days. It is now applied in the knee ligamentoplasty rehabilitation program.
