Particularity of asthmatic patients in the Sahara: Example of the Guelmim Oued Noun region, Morocco

Introduction |
L’asthme est une maladie très répandue dans le monde. Le Maroc connaît une augmentation de sa prévalence depuis quelques années (10 à 15 %). L’objectif de notre étude est de décrire le profil épidémiologique, clinique et thérapeutique chez les sujets asthmatiques de plus de 15 ans de la région de Guelmim Oued Noun.
Matériels et méthodes |
Il s’agit d’une étude transversale et descriptive étalée sur une période de 6 mois, du 1er septembre 2017 au 28 février 2018. Elle a intéressé l’ensemble des patients de plus de 15 ans consultant pour asthme dans l’une des structures hospitalières de la région. La collecte des données a été effectuée à l’aide d’un questionnaire établi en mode face-à-face avec les patients.
Résultats |
Deux cent asthmatiques ont été inclus dans l’étude avec un âge moyen de 42 ans. Les hommes représentaient 44 % des patients et les femmes 56 % (sex-ratio H/F de 0,78). Parmi nos patients, 34 % avaient un bas niveau socioéconomique et 64 % un niveau moyen. La majorité de nos patients (67 %) étaient en surpoids. Les principaux facteurs de risque étaient représentés par : l’atopie (rhinite : 82 %, conjonctivite : 76 %), le RGO : 41 %, la sinusite chronique : 33 %, l’intolérance aux AINS et aspirine : 17 %. Les facteurs déclenchant les crises retrouvés étaient principalement : les poussières (81 %), les infections (76 %), le froid et le brouillard (72 %), la fumée (66 %), l’effort (66 %), les produits caustiques (62 %) et les médicaments (10 %). La spirométrie n’a été prescrite que chez 61 % des patients, la radiographie thoracique chez 75 % et les tests cutanés chez 20 % des patients. Concernant le traitement, 16 % des patients étaient en palier I, 27 % en palier II, 38 % en palier III, 10 % en palier IV et un patient en palier V. Dix-neuf patients (10 %) ne recevaient aucun traitement. L’asthme était bien contrôlé chez 59 % des patients, partiellement contrôlé chez 32 % des patients et non contrôlé chez 9 %.
Conclusion |
Cette étude a mis en évidence une population asthmatique surtout féminine avec un surpoids fréquent, des conditions climatiques difficiles, un diagnostic spirométrique insuffisant, une mauvaise observance thérapeutique et une automédication fréquente. Ces paramètres sont modifiables avec la volonté des autorités locales, des agents de santé et des patients eux-mêmes.

Introduction |
Asthma is a very common disease around the world. Morocco has seen an increase in its prevalence for several years (10 to 15%). The objective of our study is to describe the epidemiological, clinical and therapeutic profile in asthmatic subjects over 15 years of age in the region of Guelmim Oued Noun.
Materials and methods |
This is a cross-sectional, descriptive study spread over a period of 6 months, from September 1, 2017 to February 28, 2018. It involved all patients over 15 years of age consulting for asthma in one of the hospital structures in the region. Data collection was carried out using a questionnaire established in face-to-face mode with patients.
Results |
Two hundred asthmatics were included in the study with a mean age of 42 years. Men represented 44% of patients and women 56% (sex ratio M/F 0.78). Among our patients, 34% had a low socioeconomic level and 64% an average level. Most of our patients (67%) were overweight. The main risk factors were represented by: atopy (rhinitis: 82%, conjunctivitis: 76%), GERD: 41%, chronic sinusitis: 33%, intolerance to NSAIDs and aspirin: 17%. The factors triggering the crisis found were mainly: dust (81%), infections (76%), cold and fog (72%), smoke (66%), effort (66%), caustics (62%) and drugs (10%). Spirometry was only prescribed in 61% of patients, chest X-ray in 75% and skin tests in 20% of patients. Regarding the treatment, 16% of the patients were in stage I, 27% in stage II, 38% in stage III, 10% in stage IV and one patient in stage V. Nineteen patients (10%) received no treatment. Asthma was well controlled in 59% of patients, partially controlled in 32% of patients and uncontrolled in 9%.
Conclusion |
This study highlighted a predominantly female asthmatic population, with frequent overweight, difficult climatic conditions, insufficient spirometric diagnosis, poor treatment compliance and frequent self-medication. These parameters can be modified with the will of local authorities, health workers and the patients themselves.
