Les inhibiteurs de point de contrôle immunitaire ont complètement bouleversé l’algorithme thérapeutique de plusieurs types de cancer, conférant chez certains patients un long bénéfice clinique avec un bon profil de toxicité. Le « rechallenge » est une option intéressante chez ces patients, car la chimiothérapie palliative est souvent le seul traitement alternatif avec des bénéfices très limités. Malgré l’usage récente de cette stratégie dans la pratique clinique, aucun recommandation n’est actuellement disponible pour sélectionner les bons candidats. En effet, peu d’information est rapportée sur l’efficacité et la tolérance de la réintroduction des médicaments anti-Programmed Death 1 (PD1) et anti-Programed Death Ligand 1 (PDL1). La plupart des données disponibles à ce jour proviennent de cohortes rétrospectives et d’analyses de sous-groupes dans le cadre d’essais prospectifs randomisés. Dans cette revue, nous faisons état des évidences disponibles sur l’efficacité et la sécurité du « rechallenge » avec les anti-PD1/PDL1 dans le cancer du poumon non à petites cellules (CPNPC) avancé et nous présentons les critères cliniques potentiellement utiles pour la sélection des patients qui ont été mis en évidence jusqu’à présent.

Immune checkpoint inhibitors have completely overturned the therapeutic algorithm for several types of cancer, giving to some patients a long clinical benefit with acceptable toxicity. Re-chellenge is an interesting option in these patients, as palliative chemotherapy is often the only alternative treatment with a limited benefit. Despite the recent use of this strategy into clinical practice, no reference is currently available to select the right candidates. Indeed, little is known about the efficacy and tolerance of the reintroduction of anti-Programmed Death 1 (PD1) and anti-Programmed Death Ligand 1 (PDL1) médicaments. Most of data available to date come from retrospective cohorts and subgroup analyses from prospective randomized trials. In this review, we report the available evidences about the efficacy and safety of rechellenge with anti-PD1/PDL1 in advanced non-small cell lung cancer (NSCLC) and highlight the potentially useful clinical criteria for patient selection that have been identified to date.
