Reversible corneal stromal thinning, acute-onset white cataract and angle-closure glaucoma due to erdafitinib, a fibroblast growth factor receptor inhibitor: Report of three cases - 06/01/21

To report corneal and lens toxicity in patients undergoing chemotherapy with erdafitinib, a fibroblast growth factor receptor (FGFR) inhibitor.
Methods |
This retrospective case series contains three patients from a cohort of 41 patients receiving erdafitinib, a selective pan-FGFR inhibitor, for chemotherapy. These three patients underwent complete ophthalmic examination: one was followed by corneal topography and the other two were followed by anterior segment optical coherence tomography.
Results |
All three patients had severe dry eye syndrome. One patient had bilateral corneal thinning. One patient had bilateral neurosensory retinal detachment, unilateral corneal thinning and white cataracts in both eyes. The third patient had bilateral corneal thinning, a corneal ulcer of the left eye and acute-onset white cataracts in both eyes, causing angle-closure glaucoma in the left eye. Following the cessation of erdafitinib treatment or a decrease in the dose used, corneal thinning resolved in all three cases within four months. Acute-onset cataracts were treated urgently by surgery, with no complications. In one patient, although the corneal ulcer healed, corneal transparency was lost, and the patient never fully recovered his initial vision.
Conclusion |
Bilateral neurosensory retinal detachment associated with FGFR inhibitor use has already been reported. However, we provide herein the first report of reversible corneal thinning and acute-onset white cataracts causing angle-closure glaucoma associated with FGFR inhibitor use. Early recognition and management of these adverse ocular reactions are required to prevent vision loss due to acute glaucoma and/or corneal ulcer.

Objectif |
Décrire les toxicités cornéenne et cristallinienne observées chez les patients traités par Erdafitinib, un inhibiteur du récepteur au facteur de croissance des fibroblastes (FGFR).
Méthodes |
Il s’agit d’une série rétrospective portant sur 3 patients issus d’une cohorte de 41 patients traités par Erdafitinib, un inhibiteur sélectif pan-FGFR. Les trois patients ont été suivis par un examen ophtalmologique complet, et également par topographie cornéenne pour un patient et par tomographie en cohérence optique du segment antérieur pour deux patients.
Résultats |
Les 3 patients présentaient une sécheresse oculaire sévère. Un patient a présenté un amincissement stromal cornéen bilatéral. Un patient a présenté un décollement séreux rétinien bilatéral, un amincissement stromal cornéen unilatéral et une cataracte blanche bilatérale. Le troisième patient a présenté un amincissement stromal cornéen bilatéral, un ulcère cornéen de l’œil gauche et a contracté une cataracte blanche bilatérale d’apparition aiguë, entraînant un glaucome à angle fermé de l’œil gauche. Une fois l’Erdafitinib arrêté ou les doses réduites, une résolution de l’amincissement stromal cornéen a été observée dans les trois cas dans un délai de 4 mois. Les cataractes ont été opérées sans complications. Le patient ayant présenté un ulcère cornéen a conservé une cicatrice stromale avec baisse de l’acuité visuelle persistante.
Conclusion |
La survenue de décollement séreux rétinien associé à l’utilisation d’inhibiteurs du FGFR a déjà été rapportée. En revanche la survenue d’amincissement stromal cornéen réversible, l’apparition aiguë de cataracte blanche responsable de la survenue de glaucome à angle fermé sont rapportés pour la première fois dans un contexte de traitement par inhibiteur de FGFR. Une reconnaissance et une gestion précoces de ces effets indésirables sont nécessaires pour prévenir une baisse d’acuité visuelle irréversible.
