Résumé |
Introduction |
Cet article a l’objectif de présenter les résultats d’une étude consacrée à la prise en charge thérapeutique de patients migrants présentant des troubles psychiques et pour lesquels une indication d’antipsychotique a été posée. L’étude s’intéresse à la psychopathologie présentée par les patients, au rôle de l’interprète communautaire dans l’investigation psychopathologique des patients, et au processus de prise de décision aboutissant à un diagnostic et à la prescription d’un antipsychotique.
Objectifs |
L’objet de la recherche est le processus décisionnel de l’auteure face à la complexité des situations cliniques que présentaient les patients pour lesquels elle a prescrit un antipsychotique.
Méthode |
Les données de la recherche ont été prises des dossiers cliniques des patients, suivis par l’auteure entre décembre 2013 et octobre 2017, qui ont pris au moins pendant un mois un antipsychotique. Le nombre total de dossiers considérés pour cette étude est de 16, après l’exclusion de certains patients qui n’ont pas pu donner de consentement éclairé. La méthode utilisée est une étude qualitative rétrospective du contenu des dossiers des patients.
Résultats |
Le suivi des patients de l’étude a nécessité, dans la plupart des cas, l’intervention d’un interprète communautaire. L’introduction de l’antipsychotique a eu lieu généralement dans des moments de crise. L’antipsychotique a pu être arrêté quand la qualité de vie des patients présentant des problématiques de type post-traumatiques, s’étaient améliorées. La prise en charge de certains patients (quand la dimension culturelle était un élément important à considérer) a bénéficié du rôle de médiateur culturel de l’interprète communautaire.
Conclusion |
Les résultats de cette étude pourraient contribuer à améliorer la compréhension des relations qui existent entre traumatisme, dissociation, psychose, déterminants sociaux de la santé et culture. Ils éclaireront également la nécessité de proposer des formations spécifiques au diagnostic en contexte interculturel et de vulnérabilité sociale. Cette étude permettra une prise de conscience sur la nécessité d’investiguer davantage dans ce champ de la psychiatrie, qui nécessite de différents domaines du savoir indispensables à une bonne prise en charge de ces patients : la santé publique, la psychopathologie, la psychotraumatologie, l’anthropologie médicale et les neurosciences.

Introduction |
The aim of this article is to show the results of a study of the psychiatric follow-up of migrants suffering a mental health problem that have received an antipsychotic treatment. Most consultations had taken place with an interpreter. The research is interested to investigate: psychopathology of patients taking antipsychotics, the role of interpreter in psychopathology investigation, and the decision-making process of the researcher to diagnose and prescribe an antipsychotic drug.
Objectives |
The purpose of the research is the author's decision-making process regarding the complexity of the clinical situations that presented the patients for whom she prescribed an antipsychotic.
Method |
The researcher will only investigate files from the patients who had taken antipsychotics at least for a month from December 2013 until September 2017. The final number of files studied was 16, after exclusion of files of patients that could not have given their inform consent. The research will be a retrospective qualitative study of the content of patient files.
Results |
The follow-up of the patients in the study required, in most cases, the intervention of a community interpreter. The introduction of the antipsychotic usually took place during a crisis. The antipsychotic could be stopped when the quality of life of patients with post-traumatic problems had improved. The care of some patients (when cultural dimension was an important element to consider) has benefited from the cultural mediator role of the community interpreter.
Conclusion |
Results of this study will provide a better understanding of relationships between trauma, dissociation, psychosis and cultural interpretations of illness; as well as some clues explaining prescription of antipsychotics. Results will show the need of training programs for the treatment of patients with a severe mental health problem with a vulnerable context and who have lived, in most cases, psychotraumatism. This study will raise awareness about the need to investigate more in this field of psychiatry, which requires different areas of knowledge essential for proper care of these patients: public health, psychopathology, psychotraumatology, medical anthropology and neuroscience.
