Treatment of oligorecurrent prostate carcinoma after local treatment with curative intend, in the era of metabolic imaging and stereotaxic radiotherapy

Introduction |
Dans le cancer de prostate au stade oligo-récidivant après traitement local à but curatif, une stratégie de traitement des métastases pourrait être une alternative au traitement systémique standard par suppression androgénique.
Matériel et méthode |
Nous présentons dans un cas clinique illustré, un état de l’art de la définition du stade oligométastatique, et de son traitement par radiothérapie exclusive des métastases vs une suppression androgénique.
Résultats |
Le développement de l’imagerie métabolique permet de reclasser en M1 une part croissante de patients M0 en imagerie conventionnelle, principalement au stade de récidive après traitement local curatif. Deux essais randomisés de phase II (STOMP et ORIOLE) ont démontré que dans cette situation, le traitement des métastases par radiothérapie stéréotaxique (ou chirurgie) est bien toléré et permet de retarder significativement l’initiation d’une suppression androgénique, sans pour autant modifier la survie globale.
Conclusion |
Des essais prospectifs de plus larges effectifs sont en cours pour confirmer les données préliminaires de STOMP et ORIOLE et devraient permettre d’identifier les patients oligo-récidivants pour lesquels un traitement systémique peut être différé.

Introduction |
In oligo-recurrent prostate cancer after local treatment for curative intend, stereotaxic radiotherapy of metastasis could be an alternative to standard androgen deprivation therapy (ADT).
Material and method |
In a case report, we present a state of the art of the definition of the “oligometastatic disease”, and the level of evidence to support first stereotaxic radiotherapy of metastasis vs. ADT.
Results |
The development of metabolic imaging has made it possible to reclassify as M1an increasing proportion of M0 patients with conventional imaging, mainly at the step of recurrence after local curative treatment. Two randomized phase II trials (STOMP and ORIOLE) demonstrated that in this situation, the treatment of metastases by stereotaxic radiotherapy (or surgery) was well tolerated and made it possible to significantly delay the initiation of ADT, but didn’t demonstrate an improvement in overall survival.
Conclusion |
Larger prospective trials are conducted currenly to confirm the preliminary data from ORIOLE and STOMP and should make it possible to identify oligo-recurrent patients for whom systemic treatment can be delayed.
