Les lymphomes mammaires primaires sont rares, représentant 0,04 à 0,52 % de toutes les néoplasies du sein. Leur expression clinique et radiologique est non spécifique. Nous rapportons un cas de lymphome mammaire primaire unilatéral isolé chez une patiente de 85 ans découvert au cours d’un examen sénologique de dépistage. Le couple échomammographie a révélé une masse suspecte classée 4c selon le Breast imaging-reporting and data system (BI-RADS). L’analyse de trois prélèvements microbiopsiques à l’aiguille 16 G sous guidage échographique a révélé une prolifération lymphomateuse B folliculaire de bas grade. Un traitement ciblé basé sur une irradiation externe a été proposé à la patiente, qu’elle a refusé.

Primary breast lymphomas are rare, accounting for 0.04 to 0.52% of all breast neoplasms. Their clinical and radiological expression is non-specific. We report an isolated case of unilateral primary breast lymphoma in an 85-year-old patient who was detected as a mass at mammogram and breast ultrasound examinations and categorised as 4c according to the Breast imaging-reporting and data system (BI-RADS). Three ultrasound-guided microbiopsies were taken from the breast mass using a 16 G needle. A histological study revealed a low-grade follicular B lymphomatous proliferation. The patient was offered targeted treatment with external irradiation, which she declined.
