Introduction |
On assiste actuellement à un développement important de la recherche qualitative en santé. L’application de ces approches dans la recherche clinique en psychiatrie de l’enfant et de l’adolescent est un défi méthodologique. Cet article présente une nouvelle méthode qualitative, adaptée pour la recherche clinique médicale : IPSE (Inductive Process to Analyze the Structure of lived Experience), visant à produire des recommandations concrètes pour améliorer les soins, notamment dans le champ de la psychiatrie de l’enfant et de l’adolescent.
Méthode |
L’approche IPSE comporte cinq étapes : (1) constitution du groupe de recherche ; (2) s’assurer de l’originalité de la recherche ; (3) recrutement et échantillonnage visant l’exemplarité ; (4) recueil des données pour accéder à l’expérience des participants, et 5) analyse des données. Cette dernière étape est structurée en deux phases : (i) individuelle descriptive ; (ii) groupale pour dégager une structure d’expérience, à partir de laquelle seront formulées des propositions concrètes afin d’améliorer les soins.
Résultats |
À titre d’exemple d’application d’IPSE dans le champ de la pédopsychiatrie, nous présentons une étude qualitative explorant l’alliance thérapeutique dans l’anorexie mentale. Les résultats de cette étude ont donné lieu à plusieurs implications cliniques et de recherche : l’élaboration d’un Patient Reported Ouctomes (PRO) qu’on pourrait traduire par « résultats rapportés directement par le patient », recommandations cliniques, et stratégies nouvelles pour améliorer l’alliance parents/patients/médecins.
Conclusion |
IPSE est une méthode qualitative spécifiquement développée pour la recherche clinique médicale appliquée qui peut s’intégrer à la médecine fondée sur les données probantes (Evidence Based Medicine, EBM) via des designs mixtes. Dans le champ de la pédopsychiatrie, elle permet d’explorer en profondeur les aspects subjectifs et expérientiels des pathologies pédopsychiatriques et de mieux prendre en compte le savoir expérientiel des patients et des parents dans la pratique clinique comme dans la formation aux étudiants et futurs psychiatres.

Background |
Qualitative research is now booming in biomedical clinical studies and in many medical specialties, including child and adolescent psychiatry, aimed at obtaining an in-depth understanding of phenomena within their natural setting and directly from the perspective of the people experiencing them. In the field of child and adolescent psychiatry, qualitative research can enhance better dialogue and coordination between clinicians and researchers. Our research group - including child & adolescent psychiatrists, general psychiatrists, psychologists and doctors of other specialties–has gained an expertise in the qualitative exploration of complex medical issues regarding illness and treatment. Developing and conducting qualitative studies within child & adolescent psychiatry research is a methodological challenge. This paper describes an innovative qualitative approach : Inductive Process to Analyze the Structure of lived Experience (IPSE), an inductive and experiential method designed to gain the closest access possible to the patient's lived experience and to reach concrete proposals for improving care and research.
Methods |
IPSE is a five-steps process: (1) set up a research group ; (2) ensure the originality of the research; (3) organize recruitment and sampling intended to optimize exemplarity; (4) collect data that enable entry into the subjects ‘experience’, and (5) analyze the data. This final stage is composed of one individual descriptive phase, followed by two group phases: (i) define the structure of the lived experience, and (ii) translate the findings into concrete proposals that make a difference in care.
Results |
We illustrate the application of the IPSE approach in the field of child and adolescent psychiatry using the example of a recent qualitative study exploring the therapeutic alliance among adolescents with anorexia nervosa, their parents and their psychiatrists. Results are structured around two axes, obstacles and facilitators, which enabled drawing concrete implications: development of a Patient Reported Outcome (PRO), clinical recommendations and innovative strategies to improve parents/patients/doctors alliance.
Conclusion |
IPSE is a qualitative method specifically developed for clinical medical research, particularly relevant in the field of child and adolescent psychiatry, to reach concrete proposals and then be directly integrated within evidence-based medicine. In the field of child and adolescent psychiatry, IPSE can explore in-depth experiential and subjective aspects of psychiatric disorders and better take into account the experiential knowledge of patients and parents within clinical practice and training of medical students and psychiatry trainees.
