Acceptability and results of Postpartum Proctological Consultation for Women Who Have Had an Obstetric Injury of the Anal Sphincter

Objectifs: Une prise en charge précoce de désordres périnéaux après lésion obstétricale du sphincter anal(LOSA) 3 ou 4 par une consultation proctologique pourrait améliorer le pronostic fonctionnel. L’objectif était d’évaluer l’acceptabilité et le bénéfice de cette consultation et d’en rapporter ses résultats.
Méthodes: Cette étude rétrospective unicentrique incluait 64 femmes ayant eu une LOSA 3 ou 4 et chez qui une consultation proctologique du postpartum systématique à 3 mois était proposée. Les diagnostics et modes de prise en charge proposés au décours étaient relevés.
Résultats: Parmi les 5070 femmes ayant donné naissance par voie vaginale, 64 femmes ont eu une LOSA (1,2%) parmi celles-ci 54 se sont présentées à la consultation qui leur était proposée (84,3%). À l’interrogatoire, 21 femmes n’avaient aucune plainte. Un diagnostic de troubles de l’évacuation ou de maladie hémorroïdaire notamment a été posé chez 10 femmes. Une prise en charge spécifique a été proposée 16 fois et une consultation de contrôle demandée 3 fois. Un diagnostic a été proposé chez 31 femmes parmi les 33 présentant au moins un symptôme ano-rectal ou périnéal et 27 ont bénéficié d’un traitement spécifique. Une consultation de contrôle a été proposée à 18 femmes. Les principaux diagnostics posés étaient une incontinence au gaz, une dyschésie avec ou sans asynchronisme abdominopérinéal et une diminution du tonus sphinctérien. Les principales mesures entreprises étaient la prolongation des laxatifs, la proposition de massage périnéaux et une rééducation abdomino-périnéale par biofeedback.
Conclusion: La consultation proctologique systématique après une LOSA 3 ou 4 est acceptée et permet de mettre en place une prise en charge spécifique dans la majorité des cas. L’évaluation de son impact à long terme reste à définir.

Objectives: Early management of perineal disorders after obstetric anal sphincter injury (OASI) may improve the functional prognosis. The objective was to assess the acceptability of the consultation and to report on its results.
Methods: This unicentric retrospective study included 64 women who had LOSA 3 or 4 and who were offered a 3-month routine postpartum proctological consultation. The proposed diagnoses and recoveries were identified.
Results: Of the 5,070 women who gave birth vaginally, 64 women had LOSA (1.2%). 54 came to the clinic (84.3%). At the interview, 21 women had no complaints. A diagnosis of evacuation disorders or hemorrhoidal disease in particular was made in 10 women. A specific management was proposed to 16 women and a control consultation was proposed to 3 women. Of the 33 women with at least one symptom, 31 women were diagnosed and 27 women were offered specific management. A monitoring consultation was offered to 18 women. The main diagnoses were gas incontinence, dyschesia with or without abdominopererial asynchronism and decreased sphincter tone. The main measures undertaken were the extension of laxatives, the proposal of perineal massage and abdominal-perineal rehabilitation by biofeedback.
Conclusion: Systematic proctological consultation was acceptable and allows for a specific management in the majority of cases. The assessment of its long-term impact remains to be defined.
