Voir les affiliationsLe traitement par toxine botulique de la spasticité localisée aux membres inférieurs chez l’IMC doit être associé à une prise en charge spécifique de kinésithérapie et d’appareillage du fait des particularités du produit injecté.
Le protocole de kinésithérapie et d’appareillage présenté dans ce travail vise à l’amélioration de l’équilibre global de l’enfant afin de faciliter la station debout puis l’accession à la marche autonome.
Lorsque la déambulation de l’enfant est autonome, le traitement après injection de toxine est essentiellement ciblé sur la qualité et le rendement de la marche.
L’appareillage a une place de choix dans ce traitement pour :
- récupérer les déficits de longueur (rétractions non fixées) par des attelles de posture ;
- suppléer à la faiblesse musculaire par l’appareillage fonctionnel.
Le traitement doit être en permanence adapté à l’évolution neuro-motrice et orthopédique de l’enfant.

Botulinum toxin used for the treatment of focal spasm of the lower limbs in children with cerebral palsy should be associated with a specific physical medicine and technical assistance strategy.
The physical therapy and technical assistance strategy presented here is designed to improve overall balance and thus facilitate the upright position and independent gait.
When the child is able to walk independently, treatment after toxin injection is essentially targeted to improving gait quality and yield.
Technical assistive devices are important to recover length deficits (non-fixed retraction) using posture ortheses and support weak muscles with functional devices.
Treatment must be adapted regularly to the neuromotor and orthopedic status of each child.
