Partant du principe qu’« il est beaucoup plus efficace de prévenir les cicatrices hypertrophiques que de les traiter », une évaluation précoce et correcte d’une cicatrice est nécessaire à la mise en place d’une stratégie thérapeutique.
L’évolution de ces lésions peut être longue (un an ou plus) et hasardeuse. Il faudra donc, au plus tôt, savoir quand drainer, comprimer, masser ou s’abstenir.
Les outils de l’évaluation cicatricielle sont :
- L’interrogatoire précisant d’éventuels antécédents personnels ou familiaux.
- L’examen de la lésion : aspect, consistance, coloration, expansions.
- Les dimensions de la lésion.
- Les tests d’étirement et de plissement-décollement cutané.
- Le test à la vitropression.
- L’échelle de Vancouver.
- La photographie.
- Le bilan articulaire.
- Le bilan de prurit.
- L’avis du patient exprimant son vécu de la lésion.
Ces tests et bilans rassemblés dans une fiche-bilan permettent d’apprécier l’ensemble des caractéristiques d’une cicatrice ainsi que son potentiel pathogène probable. Ce préjudice peut être soit fonctionnel par adhérence ou bride articulaire, soit esthétique par hypertrophie ou rétraction disgracieuse.

On the bases that it is much more effective to prevent hypertrophic scars than to have to treat them, an early and adequate assessment of the scar is necessary for an adequate therapeutic strategy.
The ongoing course of these lesions can be lengthy (one year or more) and uncertain.
It then becomes necessary to know, as soon as possible, when to drain, when to compress, when to massage or when to abstain from doing anything.
Tools available for assessing scars include:
- Questions to check potential personal or family history of scar healing.
- Examination of the lesion: aspect, consistency, color, expansions.
- Dimensions of the lesion.
- Skin stretching test.
- “Vitropression test” (skin coloring test after pression on the lesion).
- Vancouver scar scale.
- Photography.
- Recording range of motion of implicated joint(s).
- Pruritus test.
- Opinion of the patient expressing his feeling about the lesion.
The results of these tests and check-ups are compiled in the medical chart allowing an overall evaluation of the different characteristics of the scar and its probable pathogenic potential.
This pathogenic potential may compromise function via adherences or limitation of movement, or involve the esthetic aspect due to hypertrophy or disgraceful retraction.
