Background |
Nasopharyngeal carcinoma (NPC) is among the most frequent cancers of the head and neck in Tunisia. This cancer is often localized at the time of diagnosis. Breast metastasis within NPC is a rare clinical occurrence especially in male gender. To the best of our knowledge, our present case report describes the third case of NPC with breast metastasis in a male patient cited in the literature.
Case report |
We report a 42-year-old male who was diagnosed in Salah Azaiez Institute in Tunisia in April 2019 with advanced NPC. The clinical examination and all the explorations showed it was a metastatic cancer to the bone, to the axillary lymph nodes with the presence of a right breast mass. Histological results of the biopsy of this breast mass confirmed that it was a metastasis that originated from NPC. After a multidisciplinary team meeting and a bibliographical research, a palliative treatment was decided. The patient was treated with several lines of chemotherapy and a palliative radiotherapy on the nasopharynx and cervical lymph node areas (40Gy in 20 fractions). Radiation therapy to the breast for this patient has been scheduled but he died after follow-up of 34months following a deterioration of the general condition and the respiratory state.
Conclusion |
Breast metastasis within NPC is a rare entity, with only a few cases cited in the literature. It is characterized by a poor prognosis with a survival that does not generally exceed three years. It represents a diagnostic and therapeutic challenge for oncologists. A good clinical, histopathological and immunohistochemical (IHC) examination are necessary to make the diagnosis and subsequently adopt an appropriate treatment. To date, no consensus has yet been established for the optimal treatment of this type of metastasis. Palliative treatment, including chemotherapy and radiotherapy is the main treatment strategy for these patients.

Contexte |
Le carcinome du nasopharynx fait partie des cancers de la tête et du cou les plus fréquents en Tunisie. Ce cancer est souvent localisé au moment du diagnostic. Les métastases mammaires au sein de ce cancer sont un événement clinique rare, en particulier chez le sexe masculin. À notre connaissance, notre présent rapport de cas décrit le troisième cas de la littérature de cancer nasopharyngé avec métastase mammaire chez un patient de sexe masculin.
Observation |
Nous rapportons un homme de 42 ans qui a été diagnostiqué à l’institut Salah Azaiez en Tunisie en avril 2019 avec un cancer nasopharyngé avancé. L’examen clinique et toutes les explorations ont montré qu’il s’agissait d’un cancer métastatique à l’os, aux ganglions axillaires avec présence d’une masse mammaire droite. Les résultats histologiques de la biopsie de cette masse mammaire ont confirmé qu’il s’agissait d’une métastase d’origine nasopharyngée. Après une réunion en concertation pluridisciplinaire et une recherche bibliographique, un traitement palliatif a été décidé. Le patient a été traité par plusieurs lignes de chimiothérapie et une radiothérapie palliative sur le nasopharynx et les ganglions cervicaux (40Gy en 20 fractions). Une radiothérapie du sein a été programmée pour ce patient mais il est décédé après un suivi de 34 mois suite à une détérioration de l’état général et de l’état respiratoire.
Conclusion |
La métastase mammaire au sein de cancer nasopharyngé est une entité rare, avec seulement quelques cas cités dans la littérature. Elle se caractérise par un mauvais pronostic avec une survie qui n’excède généralement pas trois ans. Elle représente un défi diagnostique et thérapeutique pour les oncologues. Un bon examen clinique, histopathologique et immunohistochimique est nécessaire pour poser le diagnostic et adopter par la suite un traitement adapté. À ce jour, aucun consensus n’a encore été établi pour le traitement optimal de ce type de métastase. Le traitement palliatif, y compris la chimiothérapie et la radiothérapie, est la principale stratégie de traitement pour ces patients.
