Résumé |
Objectif. - Évaluer la prise en charge préhospitalière par des équipes Smur des personnes âgées, la concordance entre les diagnostics préhospitaliers et hospitaliers et observer le devenir des patients après leur hospitalisation.
Type d'étude. - Rétrospective.
Patients et méthode. - Tous les patients âgés de 65 ans ou plus, pris en charge hors transfert interhospitalier, ont été inclus. Étaient recueillis les données démographiques, le motif de départ, le temps de médicalisation, la gravité initiale, la mise en condition, la destination, les diagnostics préhospitalier et hospitalier et le devenir du patient. Trois groupes ont été définis : G1 (65-74 ans), G2 (75-84 ans), G3 (> 84 ans). L'analyse statistique a été réalisée par une Anova pour les variables quantitatives et un test de 2 pour les variables qualitatives.
Résultats. - Deux cent soixante et onze patients, d'âge moyen 80 ± 8 ans ont été inclus. La mortalité préhospitalière était de 12 %. Les prises en charge étaient suivies d'un transport médicalisé dans 82 % des cas et 44 % des patients étaient adressés en réanimation. Les patients très âgés étaient significativement moins souvent admis en réanimation. Il n'existait pas de différence significative entre les 3 groupes en terme de mise en condition. Huit pour cent des patients étaient intubés. Les diagnostics préhospitaliers étaient concordants avec les motifs de départ dans 61 % des cas et avec les diagnostics intrahospitaliers dans 85 % des cas. Le devenir des patients n'était pas différent entre les 3 groupes : 53 % des patients hospitalisés retournaient à domicile.
Conclusion. - L'analyse du devenir des personnes âgées confirme l'idée que l'âge ne devrait pas influencer la décision médicale et le degré de médicalisation préhospitalière.

Objective. - To evaluate prehospital management of elderly patients, agreement between prehospital and hospital diagnosis and to observe clinical course during hospitalization.
Type of study. - Retrospective study.
Patients and method. - Out-of hospital patients of 65-year-old or more were included. Apart from demographic data, were collected: reasons for call, medicalization length, SAPS score, prehospital management, destination, prehospital and hospital diagnosis and patients evolution. Three groups were defined: G1 (65-74), G2 (75-84), G3 (> 84 year old). Statistical analysis was done byan Anova for quantitative data and bya Chi squared test for qualitative data.
Results. - Two hundred and seventy-one patients were included (mean age 80 ± 8 years, 43% of men). Eighty-two per cent of interventions were followed by a medicalized transport. Twelve per cent of patients died in the field. Forty-four per cent were hospitalised in intensive care unit, but patients of more than 84 year-old were significantly less often admitted in intensive care unit. There was no difference between the three groups in term of degree medicalization during transport. Eight per cent of patients required tracheal intubation in the field. Prehospital diagnoses were in agreement with reason for call in 61% of patients and with inhospital diagnosis in 85% of patients. Fifty three per cent of patients came back home after hospitalisation.
Conclusion . - Analysis of elderly patient evolution after hospitalisation confirms the idea that the age should not influence the decision and the degree of prehospital medicalization.
