Vascularite cérébrale avec sténoses et ectasies due au virus varicelle-zona au cours de l’infection par le VIH : une nouvelle entité compliquant le sida - 23/04/08

Résumé |
Nous rapportons trois cas d’angiopathie cérébrale avec sténoses et ectasies dus à une infection méningée par le virus varicelle-zona au cours du sida. Les tableaux cliniques étaient stéréotypés : immunodépression sévère due à l’infection par le VIH, maladie cérébrovasculaire ischémique multifocale responsable d’infarctus territoriaux, méningite lymphocytaire normoglycorachique dans deux cas, hypoglycorachique dans un cas, angiopathie multifocale des artères cérébrales de gros et moyen calibre comportant à la fois des sténoses et des ectasies, guérison avec ou sans séquelles après traitement par aciclovir. La responsabilité du virus varicelle-zona était prouvée dans les trois cas par la positivité de la PCR VZV dans le liquide cérébrospinal (LCS). Une biopsie cérébrale a été effectuée dans un cas montrant la présence du VZV au sein des lésions vasculaires et l’absence d’autre cause. Bien que le virus varicelle-zona soit connu comme cause d’atteintes vasculaires intracrâniennes, il s’agit de la première description d’une angiopathie particulière, associant sténoses et ectasies. La cause infectieuse et son caractère évolutif en l’absence de traitement imposent de l’envisager rapidement en cas de pathologie cérébrovasculaire chez un patient immunodéprimé infecté par le VIH. Le traitement antiviral par aciclovir (30mg/kg par jour, 15 jours) a été efficace dans les trois cas.

We describe three cases of cerebral angiopathy with aneurysms caused by a meningeal varicella-zoster virus infection occurring during AIDS. The clinical picture was rather stereotyped: severe immunocompromission due to HIV infection, ongoing multifocal cerebrovascular disease with territorial infarcts, lymphocytic meningitis with normal glucose content (two cases) or hypoglycorrhachia (one case), multifocal cerebral vasculopathy with narrowings and aneurysms, healing with or without neurological sequelae after intravenous aciclovir treatment. The diagnosis of varicella-zoster virus-induced angiopathy was ascertained by the positive specific PCR in the CSF in the three cases and by the results of the cerebromeningeal biopsy in one case. Although, varicella-zoster virus is already known as a cause of cerebral angiopathy both in the immunocompetent and the immunocompromised, these three cases are the first ever described of a particular angiopathy with narrowings and ectasias complicating AIDS. The infectious treatable cause and the risk of aggravation without treatment require early active oriented investigations in case of a patient with cerebrovascular disease occurring during HIV infection, including a CSF study with varicella-zoster PCR, to allow specific antiviral treatment. In our three cases, aciclovir intravenous treatment (30mg/kg per day) enabled VZ virus clearing from the CSF and stopped the course of the vasculopathy.
