Septic arthritis of the fingers: is short-term oral antibiotic therapy appropriate? - 01/05/22

The management of septic arthritis of the hand is poorly described and there is no consensus. It is based on how septic arthritis of the large joints is managed, despite certain specificities related to the hand, typically involving inpatient management and intravenous antibiotic therapy. The primary objective of this work was to evaluate our postoperative protocol for the management of septic arthritis of the hand with short-term antibiotic therapy. The secondary objective was to determine the risk factors for treatment failure. We conducted a retrospective, descriptive, single-center study and analyzed the medical records of patients managed for septic arthritis of the fingers over a 1-year period from January 2018 to December 2018. We collected patient demographics and all pre-, intra-, and postoperative data. A total of 128 patients were included. The median age was 52.4 years (41–66). An exogenous source of contamination was reported in 98% of cases (animal bite, plant thorn, wound, cyst trituration, etc.). The most frequently isolated microorganism was Staphylococcus aureus (45%), followed by Streptococcus spp. (22%) and Pasteurella spp. (18%). The vast majority of patients (79%) were treated with oral amoxicillin/clavulanic acid. This treatment was continued in 91% of patients after microbiological results were obtained for a median treatment duration of 8 days (7–15). Nine percent of patients failed treatment. The risk factors identified were crush wounds (p = 0.04), initial radiological abnormalities (chondrolysis and/or osteolysis) (p = 0.016) and infection with Pasteurella spp. (p = 0.015). Our study suggests that simplified antibiotic therapy is feasible for the management of septic arthritis of the hand with short duration, broad spectrum oral antibiotics in the absence of identified risk factors.

La prise en charge des arthrites septiques de la main est peu décrite et ne fait pas l’objet de consensus. Elle est dérivée de celle des arthrites des grosses articulations, malgré certaines spécificités liées à la main, avec habituellement une prise en charge en hospitalisation et antibiothérapie intraveineuse. L’objectif principal de ce travail était d’évaluer notre protocole postopératoire d’antibiothérapie per os de courte durée dans la gestion des arthrites septiques de la main. L’objectif secondaire était de déterminer les facteurs de risque d'échec infectieux à court terme. Nous avons réalisé une étude rétrospective, descriptive, monocentrique et analysé les dossiers médicaux des patients pris en charge pour une arthrite septique des doigts sur une période d’un an de janvier 2018 à décembre 2018. Nous avons colligé les données démographiques des patients et l’ensemble des données pré-, per- et post-opératoires. Au total, 128 patients ont été inclus. L'âge médian était de 52,4 ans (41–66). La contamination était exogène dans 98% des cas (morsures d’animaux, épines végétales, plaies, trituration de kyste, etc.). Le micro-organisme le plus fréquemment isolé était le Staphylococcus aureus (45%), suivi de Streptococcus spp. (22%) et de Pasteurella spp (18%). La grande majorité des patients (79%) avait reçu un traitement par amoxicilline/acide clavulanique par voie orale. Pour 91% d’entre eux, ce traitement avait été poursuivi après obtention des résultats microbiologiques pour une durée médiane de traitement de 8 jours (7–15). Neuf pour cent des patients ont présenté un échec au traitement et nous avons identifié comme facteurs de risque les plaies par écrasement (p = 0,04), les lésions ostéochondrales identifiées à la radiologie (chondrolyse et/ou ostéolyse) (p = 0,016) et les infections à Pasteurella spp. (p = 0,015). Notre étude montre la possibilité d’une antibiothérapie simplifiée dans la prise en charge de l’arthrite septique de la main avec une antibiothérapie orale de courte durée et à large spectre, en l’absence des facteurs de risque identifiés.
