Challenges in the management of cryptococcal meningitis in a pregnant woman infected with HIV: About a case in neurology from Cocody University Hospital

Introduction |
La méningite cryptococcique est rare chez les femmes enceintes infectées par le virus de l’immunodéficience humaine (VIH). La grossesse, considérée comme une période d’immunosuppression relative qui prévient le rejet fœtal, pourrait expliquer la survenue d’infections opportunistes.
Objectif |
Rapporter un traitement anticryptococcique chez une femme enceinte infectée par le VIH
Observation |
Une femme enceinte de 37 ans, infectée par le VIH-1 inobservante à son traitement antirétroviral, a été admise au service de neurologie pour des céphalées d’évolution progressive sur 3 semaines. L’examen clinique suggérait une méningo-encéphalite chez une gestante parvenue à neuf semaines d’aménorrhée. L’examen du liquide cérébro-spinal a isolé des spores de Cryptococcus Neoformans. La patiente a bénéficié selon l’essai ACTA (Advancing Cryptococcal Meningitis Treatment for Africa) de l’association fluconanzole-5 fluoro cytosine 1500mg (phase d’induction) pendant 14jours, fluconazole 1200mg (phase de consolidation et d’entretien) avec des ponctions lombaires soustractives et d’autres traitements symptomatiques. L’évolution de cette affection sur la grossesse a été favorable jusqu’à expulsion à terme d’un nouveau-né vivant hypotrophe sans malformations.
Discussion |
La prise en charge a nécessité d’évaluer les risques associés au traitement antifongique pendant la grossesse y compris la tératogénicité des agents antifongiques, leur pharmacocinétique variable pendant la grossesse, en l’absence de durée de traitement bien définie pour de telles infections et et en tenant compte des facteurs liés à l’hôte, y compris l’observance. Elle a été traitée avec succès, avec une issue fœtale favorable.
Conclusion |
La prise en charge de la cryptococcose neuroméningée chez la femme enceinte quel que soit le trimestre de grossesse doit respecter les recommandations actuelles.

Introduction |
Cryptococcal meningitis is rare in pregnant women with HIV infection. Pregnancy considered as a period of relative immunosuppression, which probably prevents fetal rejection, could explain the occurrence of opportunistic infections. We report the case of a pregnant woman infected with HIV-1.

To report anticryptococcal treatment in a pregnant woman infected with HIV
Observation |
A 37-year-old pregnant woman, infected with HIV-1 who failed to comply with her ARV treatment, was admitted to the neurology department for headache that progresses over 3 weeks. Clinical examination concluded that a pregnant woman had meningoencephalitis at 09 weeks with amenorrhea. Examination of the cerebrospinal fluid isolated Cryptococcus Neoformans spores. According to the ACTA trial (Advancing Cryptococcal Meningitis Treatment for Africa), the patient received the combination of fluconanzole-5 fluoro cytosine 1500mg (induction phase) for 14 days, fluconazole 1200mg (consolidation and maintenance phase) with subtractive lumbar punctures and other symptomatic treatments. The course of this condition during pregnancy was favorable until the expulsion at term of a living hypotrophic newborn without malformations.
Discussion |
Management required highlighting the treatment of the disease and some of the challenges associated with antifungal therapy during pregnancy including the teratogenicity of antifungal agents, their variable pharmacokinetics during pregnancy, lack of a well-defined duration of treatment for de such infections and host factors, including adherence. She was treated successfully with a favorable fetal outcome.
Conclusion |
The management of neuromeningeal cryptococcosis in pregnant women regardless of the trimester of pregnancy should comply with current recommendations.
