Recherche d’une association entre rupture des veines ponts et syndrome du bébé secoué, et comparaison des hématomes sous-duraux par secouement ou autre étiologie.
Matériel et méthodes |
Étude rétrospective et prospective avec évaluation autopsique des veines ponts chez les enfants ayant un hématome sous-dural, dans un contexte de secouement ou autre étiologie, notamment de traumatisme crânien à forte énergie cinétique.
Résultats |
Cinquante-sept enfants ayant un HSD ont été inclus, 46 bébés secoués et 11 témoins. L’âge moyen était de 6,7mois chez les bébés secoués, 43,8mois chez les témoins (p=0,07). L’hématome était plurifocal dans 74 % des bébés secoués, 50 % des témoins avec hématome sous-dural (p=0,34). Chez les bébés secoués, l’hématome était macroscopiquement supérieur à quatre jours à 64 %, et histologiquement d’âge différent à 48 %. Les veines ponts étaient rompues pour 97 % des bébés secoués, 17 % des témoins (un cas traumatique très violent) (p=0,0002).
Conclusion |
Il existe une association significative entre rupture des veines ponts et syndrome du bébé secoué. L’hématome sous-dural en cas de secouement est souvent plurifocal et d’âge différent, en faveur de secouements répétés.

Objective |
Research of an association between break of bridging veins and shaken baby syndrome, and comparison of the subdural hematoma by shaking or by an other etiology.
Methods |
Retrospective and prospective study with autopsy evaluation of bridging veins of children with subdural hematoma, in context of shaking or other etiology, notably head trauma at high cinetic energy.
Results |
Fifty-seven child have been included, 46 shaken baby syndrome and 11 witness. The mean age was 6.7month for shaken baby syndrome, 43.8month for witness (P=0,07). Hematoma was plurifocal for 74% of shaken baby syndrome, 50% for witness with subdural hematoma (P=0,34). For shaken baby syndrome, hematoma was macroscopically evaluated at more than four days for 64% and histologically of different ages at 48%. Bridging veins were broken for 97% of shaken baby syndrome, 17% of witness (a case of head trauma very violent) (P=0,0002).
Conclusion |
There is a significative association between bridging veins broken and shaken baby syndrome. The subdural hematoma in case of shaking is often plurifocal and of different ages, in favor of repeated shaking.
