Les pathologies touchant la glande corticosurrénale peuvent être dues à une augmentation ou à une diminution de la sécrétion des hormones stéroïdes que celle-ci produit, et notamment du cortisol. Le syndrome du Cushing est lié à un excès de production, alors que l’insuffisance surrénalienne est causée par un défaut de cette production. Comme la majorité des signes cliniques et biologiques sont non spécifiques, le diagnostic de ces pathologies est souvent retardé. Ces différentes pathologies peuvent avoir pour origine la glande surrénale elle-même, avec un dysfonctionnement de la synthèse des hormones stéroïdes ou être dues à une atteinte de l’axe hypothalamo-hypophysaire, conduisant à une variation de la sécrétion de l’hormone corticotrope hypophysaire (ACTH) qui régule la production de cortisol. Pour déterminer l’origine de ces pathologies, leur exploration biologique étudie en premier lieu la sécrétion du cortisol, puis de l’ACTH, pour en préciser l’origine surrénalienne ou hypothalamo-hypophysaire. Différents tests dynamiques de stimulation ou de freination sont également utilisés pour déterminer l’origine étiologique de la maladie.

Pathologies affecting the adrenocortical gland may be due to an increase or a decrease in the secretion of the steroid hormones that produces, in particular cortisol. Cushing’s syndrome is associated to an excess production, while adrenal insufficiency is caused by a defect in this production. As the majority of clinical and biological signs are non-specific, the diagnosis of these pathologies is often delayed. These different pathologies can originate from the adrenal gland itself, with a dysfunction in the production of steroid hormones, or can be due to an alteration in the hypothalamic-pituitary axis. This leads to a variation in the secretion of adrenocorticotropin hormone (ACTH) which regulates cortisol production. To determine the origin of these pathologies, their biological exploration first assessed the secretion of cortisol, then of ACTH to specify their adrenal or hypothalamic-pituitary origin. Various dynamic stimulation or suppression tests are then used to determine the etiological origin of the disease.
