The therapeutic responses to I-131 ablation in patients of differentiated thyroid carcinoma complicated with nodular goiter - 04/08/22

Introduction |
We applied the response to therapy reclassification system (RTRS) to evaluate the disease status after surgery and I-131 therapy in differentiated thyroid carcinoma (DTC) patients with nodular goiter (NG).
Materials and methods |
A total of 268 DTC complicated with NG patients who underwent the I-131 treatment and thyroidectomy between 2010 and 2018 were analyzed. The RTRS were classified into excellent (ER), indeterminate (IDR), biochemical incomplete (BIR) and structural incomplete response (SIR). Logistic regression analysis were performed to evaluate the relevant clinicopathologic and laboratory variables in the prediction of non-ER (IDR, BIR and SIR).
Results |
In the logistic analysis, gender (OR: 3.543, P=0.01), lateral cervical lymph node metastases (N1b) (OR: 6.646, P=0.005), pre-ablation stimulated thyroglobulin (Ps-Tg) (OR: 0.859, P=0.000), and anti-Tg antibody (TgAb) (OR: 64.546, P=0.000) were predictor of non-ER. The cut-off value of ps-Tg for predicting the ER was 19.98ng/mL with a sensitivity of 92.6% and specificity of 83.2%. The non-ER rate of N1b group was significantly higher than the central cervical LNM (N1a) group.
Conclusion |
For patients with DTC complicated with NG, the cut-off value of ps-Tg for predicting the ER was 19.98ng/mL. N1b patients showed inferior responses to I-131 therapy compared to N1a patients.

Nous avons appliqué le système de reclassification de la réponse au traitement (RTRS) pour évaluer l’état de la maladie après la chirurgie et le traitement par I-131 chez les patients atteints de carcinome thyroïdien différencié (DTC) et présentant un goitre nodulaire (NG).
Matériels et méthodes |
Un total de 268 patients atteints de DTC compliqué de NG qui ont subi le traitement par I-131 et une thyroïdectomie entre 2010 et 2018 ont été analysés. Les RTRS ont été classées en réponse excellente (ER), indéterminée (IDR), incomplète biochimique (BIR) et incomplète structurelle (SIR). Des analyses de régression logistique ont été réalisées pour évaluer les variables clinicopathologiques et de laboratoire pertinentes dans la prédiction de la non-ER (IDR, BIR et SIR).
Résultats |
Dans l’analyse logistique, le sexe (OR : 3,543, p=0,01), les métastases des ganglions lymphatiques cervicaux latéraux (N1b) (OR : 6,646, p=0,005), la thyroglobuline stimulée par la pré-ablation (Ps-Tg) (OR : 0,859, p=0,000) et les anticorps anti-Tg (TgAb) (OR : 64,546, p=0,000) étaient des facteurs prédictifs de non-ER. La valeur seuil de ps-Tg pour prédire le RE était de 19,98ng/mL avec une sensibilité de 92,6 % et une spécificité de 83,2 %. Le taux de non-RE du groupe N1b était significativement plus élevé que celui du groupe LNM cervical central (N1a).
Conclusion |
Pour les patients atteints de DTC compliqué de NG, la valeur seuil de ps-Tg pour prédire le ER était de 19,98ng/mL. Les patients N1b ont montré des réponses inférieures à la thérapie I-131 par rapport aux patients N1a.
