Mammoplasty for hypotrophic tuberous breast augmentation using implants. One-stage surgical strategy without cutaneous-glandular dissociation

Parmi les malformations congénitales du sein, l’incidence des seins tubéreux est très élevée, l’anomalie de forme est souvent accompagnée d’une hypoplasie de la glande mammaire et d’une asymétrie. Le retentissement psychologique de cette malformation est particulièrement important chez ces jeunes filles au décours de la puberté, en transit entre l’adolescence et l’âge adulte. L’indication d’augmentation par implants doit être systématiquement associée à un remodelage du sein afin d’obtenir une bonne adéquation sein-prothèse. L’augmentation de volume du sein par implant demeure pertinente lorsque les réserves adipeuses sont insuffisantes pour envisager un auto-transfert graisseux. En revanche, L’augmentation par implants est un véritable défi pour obtenir un bon résultat et nécessite de penser et comprendre l’anomalie afin de suivre une stratégie rigoureuse qui traite étape par étape les différents problèmes. C’est cette stratégie que nous allons développer avec une approche originale de la redistribution glandulaire et de la gestion de la procidence aréolaire. Cette stratégie est illustrée par une série de 145 patientes opérées entre 2000 à 2021.

Among congenital breast malformations, the incidence of tuberous breasts is large, the shape abnormality is often accompanied by hypoplasia of the mammary gland and asymmetry. The psychological impact of this malformation is particularly important in these young girls at the end of puberty, in transit between adolescence and adulthood. The indication for augmentation by implants must be systematically associated with breast remodeling in order to obtain a good fit between the breast and the prosthesis. The increase in breast volume by implant remains relevant when fat reserves are insufficient to consider fat self-transfer. On the other hand, Augmentation by implants is a real challenge to obtain a good result and requires thinking and understanding the anomaly in order to follow a rigorous strategy that treats the different problems step by step. It is this strategy that we will develop with an original approach to glandular redistribution and management of areolar prolapse. This strategy is illustrated by a series of 145 patients operated between 2000 to 2021.
