La cyamémazine est l’antipsychotique le plus prescrit en France, souvent en association avec un autre antipsychotique, pour sa composante sédative et anxiolytique. Mettre à disposition du clinicien des concentrations sériques de cyamémazine dans différents contextes (vérification de l’observance, inefficacité, effets indésirables, intoxication, modification des paramètres pharmacocinétiques…) nécessite de pouvoir interpréter à chaque fois celles-ci. Cet article fait le point sur la façon d’interpréter une concentration de cyamémazine, qui reste peu documentée. L’effet anxiolytique apparaît pour des concentrations sériques résiduelles à l’équilibre de 4 à 5μg/L ; l’effet antipsychotique à partir de 18–20μg/L. La cyamémazine n’est pas un médicament à marge thérapeutique étroite et des concentrations jusqu’à 400μg/L peuvent être recherchées en cas d’efficacité partielle ; des concentrations de 1800μg/L sont potentiellement mortelles ; des concentrations plus faibles pourraient s’avérer mortelles en cas d’associations avec d’autres médicaments à fortes concentrations.

Cyamemazine is the most prescribed antipsychotic drug in France, often in combination with another antipsychotic, for its sedative and anxiolytic component. Providing to physicians serum concentrations of cyamemazine in different contexts (compliance checking, ineffectiveness, adverse effects, intoxication, modification of pharmacokinetic parameters…) requires to interpret them correctly. This article presents an update on how to interpret a concentration of cyamemazine, wich remains poorly documented. The anxiolysis occurs at steady-state serum trough concentrations of 4 to 5μg/L; the antipsychotic effect from 18–20μg/L. Cyamemazine is not a drug with a narrow therapeutic window and concentrations up to 400μg/L may be sought in cases of partial efficacy; concentrations of 1800μg/L might be fatal; lower concentrations might be fatal if association with high others concentrations of drugs.
