Fragility and anesthesia in the elderly

z Le nombre de patients âgés subissant une anesthésie ne cesse d’augmenter z Plus que l’âge physiologique, c’est le terrain et l’existence d’un syndrome de fragilité qui conditionnent le pronostic z L’évaluation préanesthésique doit prendre en compte ces éléments, avec une attention particulière portée sur l’appréciation des comorbidités, des réserves physiologiques et sur la détection d’éventuels troubles cognitifs en préopératoire z La prise en charge anesthésique doit être particulièrement vigilante afin d’éviter les surdosages z La titration du remplissage vasculaire, une ventilation protectrice et le monitorage de la profondeur de l’anesthésie sont recommandés, en particulier pour les chirurgies à risque z Plus que le choix du type d’anesthésie, générale ou locorégionale, l’optimisation des techniques semble le point primordial.

The number of elderly patients undergoing anesthesia is constantly increasing. More than the physiological age, it is the terrain and the existence of a frailty syndrome that condition the prognosis. The preanesthetic evaluation must take into account these elements, with a particular attention paid on the evaluation of comorbidities, of physiological reserves and the detection of possible cognitive disorders preoperatively. Anesthetic management must be particularly vigilant in order to avoid overdosing. The titration of vascular filling, protective ventilation and monitoring of the depth of anesthesia are recommended, in particular for high-risk surgery. More than the choice of the type of anesthesia, general or locoregional, the optimization of the techniques seems to be the primordial point.
