A focus on Incontinence-associated dermatitis

La Dermite Associée à l’Incontinence (DAI) est une inflammation de l’épiderme secondaire à une exposition prolongée des régions périnéales et anales aux urines et/ou aux selles. C’est une pathologie méconnue et pourtant fréquente. Son diagnostic est difficile et sa clinique souvent confondue avec d’autres pathologies cutanées telles que l’escarre, la mycose ou encore l’eczéma de contact. Une prise en charge de l’incontinence, notamment par des changes plus fréquents, permet de limiter le temps de contact de la peau avec les urines et les selles, et constitue ainsi un élément clé de son traitement. Doivent s’y associer des soins locaux comprenant un lavage à l’eau et au savon neutre, l’application d’un agent hydratant et l’utilisation d’un protecteur cutané. La formation des soignants au diagnostic clinique de la DAI et à l’identification de ses facteurs de risque est nécessaire à une meilleure prévention et à un diagnostic plus précoce de cette pathologie cutanée.

Incontinence-Associated Dermatitis (IAD) is an inflammation of the epidermis secondary to prolonged exposure of the perineal and anal regions to urine and/or stools. It is a little-known and yet common pathology. Its diagnosis is difficult and its clinical presentation is often confused with other skin pathologies, such as pressure ulcer, mycosis or contact eczema. The management of incontinence, in particular through more frequent diaper changes, makes it possible to limit the duration of contact of the skin with urine and stools, and thus constitutes a key element in treatment of this condition. This should be associated with local care, including washing with water and neutral soap, the application of a moisturizing agent and the use of a skin protector. Training of caregivers in the clinical diagnosis of IAD and the identification of its risk factors is needed for better prevention and earlier diagnosis of this skin pathology.
