Évaluer la réactivité du nouveau dosage de prolactine Elecsys® prolactin II (PRLII) avec la macroprolactine.
Matériel et méthodes |
Nous avons comparé les résultats de deux dosages de prolactine Elecsys® (Roche Diagnostics), l’ancien prolactin (PRL) et le nouveau prolactin II dans 213 sérums hyperprolactinémiques d’après le résultat PRL : 102 négatifs en macroprolactine d’après la précipitation au polyéthylène glycol (PEG) avec le dosage PRL (MPRL<50 %), 87 positifs (MPRL≥60 %) et 24 indéterminés. La prolactine monomérique a été déterminée avec le dosage PRL (PRLm) dans tous les sérums et avec le dosage PRLII (PRLIIm) dans 142 sérums sur les 144 ayant un résultat PRLII élevé.
Résultats |
Dans le groupe négatif, les résultats PRL et PRLII sont parfaitement corrélés (r=0,998) mais le résultat PRLII est de 20 à 30 % inférieur au résultat PRL. Dans le groupe positif, le dosage PRLII apparaît moins sensible que le dosage PRL à la macroprolactine (49 sérums avec PRL élevée ont une PRLII normale) mais conserve cependant une certaine réactivité avec la macroprolactine (parmi 82 sérums avec une PRLm normale 33 ont une PRLII élevée). Sur les 142 sérums étudiés la corrélation (r=0,98) et la concordance (95 %) entre les résultats PRLm et PRLIIm sont bonnes.
Conclusion |
Le dosage prolactin II réagit moins fortement avec la macroprolactine que le dosage prolactin. Cependant, la précipitation au PEG associée à la détermination de la prolactine monomérique reste indispensable pour confirmer une hyperprolactinémie.

Aim of the study |
To evaluate the reactivity of the new Elecsys® prolactin II (PRLII) immunoassay with macroprolactin.
Material and methods |
We compared the results of the two Elecsys® prolactin assays from Roche Diagnostics, the old prolactin (PRL) and the new prolactin II in 213 hyperprolactinemic sera as measured with the PRL assay. Macroprolactin was determined after polyethylene glycol (PEG) precipitation using the PRL assay (MPRL). One hundred and two sera were considered as macroprolactin negative (MPRL<50%), 87 positive (MPRL≥60%) and 24 undetermined. Monomeric prolactin was measured with the PRL assay (PRLm) in all samples and with the PRLII assay (PRLIIm) in 142 from the 144 samples with increased PRLII values.
Results |
In the negative group, PRL and PRLII results were perfectly correlated (r=0.998) but PRLII values were 20–30% lower than PRL values. In the positive group, macroprolactin was detected to a lesser extent by the PRLII assay than by the PRL assay (49 sera with increased PRL results had PRLII results within the reference interval) but the PRLII assay was still reactive with macroprolactin (among 82 sera with PRLm within the reference interval 33 had increased PRLII results). Correlation (r=0.98) and concordance (95%) between PRLm and PRLIIm values determined in 142 sera were good.
Conclusion |
The prolactin II assay reacts less strongly than the prolactin assay with macroprolactin. However, the PEG precipitation and monomeric prolactin determination are still required to confirm hyperprolactinemia.
