Bien-fondé de l’anesthésie locale pour la pose de bandelettes sous-urétrales dans la cure d’incontinence urinaire chez la femme - 19/05/08

Objectif |
L’objectif de cette étude est d’évaluer la faisabilité et le confort de l’anesthésie locale plus ou moins associée à une sédation légère, pour la pose de bandelettes sous-urétrales dans la cure d’incontinence urinaire.
Matériel et méthodes |
Dans une série continue sans sélection, 138patientes, bénéficiant de la pose isolée de bandelette sous-urétrales (51 : Monarc® ; 18 : TVT-O® et 69 : TVTSecur®), sont soumises, après prémédication (alpazolam, hydroxyzine), à une anesthésie locale pure (ropivacaïne–clonidine et adrénaline) pour le TVTSecur® et associée à une sédation légère (midazolam–sufentanil) pour le TVT-O® et Monarc®. L’anxiété, la douleur peropératoire et la satisfaction des patientes ont été évaluées par une échelle visuelle analogique (EVA).
Résultats |
Aucune différence significative n’est mise en évidence pour l’anxiété préopératoire dans les trois groupes, de même pour la douleur peropératoire classée en trois groupes EVA : 1–30, 40–60 et 70–100mm avec, respectivement, pour Monarc® 54,9, 35,3 et 7,8 %, pour TVT-O® 77,8, 22,2 et 0 % et pour TVTSecur® 47,8, 43,5 et 8,7 %. Dans la période postopératoire, des antalgiques ont été prescrits pour 92,2 et 94,4 % des Monarc® et TVT-O®, et pour 7,2 % des TVTSecur®. À plus de 92 %, les patientes recommandent ce type d’anesthésie.
Discussion |
Contrairement à la majorité des auteurs qui utilisent une sédation profonde non vigile, les faibles doses que nous avons utilisées permettent une parfaite vigilance. L’intérêt de la ropivacaïne associée à la clonidine permet à la fois un effet vasoconstricteur et une prolongation de l’effet analgésique avec un moindre effet toxique que la lidocaïne et la bupivacaïne.
Conclusion |
Nous militons pour un retour à l’anesthésie minimale invasive, comme celle décrite initialement par les promoteurs du tension-free vaginal tape (TVT), qui permet la pose de bandelettes sous-urétrales, sous anesthésie locale avec sédation légère, en ambulatoire de façon confortable pour les patientes.

To evaluate the feasibility and the comfort of the local anesthesia more or less associated with a light sedation for the sub-urethral vaginal tape in the urinary incontinence surgery.
Material and methods |
In a series without selection, 138 patients, benefiting from the isolated sub-urethral vaginal tape surgery (51: Monarc®; 18: TVT-O® and 69: TVTSecur®), are submitted, after premedication (alpazolam, hydroxyzine), to a pure local anesthesia (ropivacaine–clonidine and adrenalin) for the TVTSecur® and associated with a light sedation (midazolam–sufentan) for the TVT-O® and Monarc®. The anxiety, the peroperative pain and the satisfaction of patients have been evaluated by an analogical visual scale (EVA).

No significant difference is put in obviousness for the preoperative anxiety in three groups, similarly for the peroperative pain classified in three group, EVA: 1–30, 40–60 and 70–100mm with, respectively, for Monarc® 54.9, 35.3 and 7.8%, for TVT-O® 77.8, 22.2 and 0% and for TVTSecur® 47.8, 43.5 and 8.7%. In the postoperative period, analgesics have been prescribed for 92.2 and 94.4% of Monarc® and TVT-O® and for 7.2% of TVTSecur®. More than 92% of patients recommend this type of anesthesia.
Discussion |
Contrarily to the majority of authors that use a deep sedation, the weak dose that we have used allows a perfected vigilance. The interest of ropivacaine associated with clonidine was both to have a vasoconstrictor effect and a prolongation of the analgesic effect with a lesser toxic effect than lidocaine and bupivacaine.
Conclusion |
We militate for a return to a minimal invasive anesthesia as that described initially by promoters of the tension-free vaginal tape (TVT), that allows the sub-urethral vaginal tape surgery, under local anesthesia with light sedation, in ambulatory of comfortable manner for patients.
