Résumé |
Les Infections du site opératoire (ISO) restent des complications fréquentes de la chirurgie : leur taux mesuré dans le cadre d'études se situe entre 2 et 5 %. La létalité des ISO est de l'ordre 2 %, mais après certaines chirurgies elle peut atteindre 30 %. Certaines de ces infections ont parfois des conséquences fonctionnelles dévastatrices. Elles s'accompagnent d'une augmentation moyenne de la durée de séjour de 7 à 10 jours, pouvant atteindre 20 à 30 jours pour les infections graves. Cette étude portant sur les salles de chirurgie digestive, urologique et orthopédique s'inscrit dans le cadre d'une recherche de conception optimale de restructuration des blocs opératoires. En arrière plan de cette étude, la norme en cours ou en projet définit les notions des zones à risques et de maîtrise des contaminations aéroportées. L'étude menée propose une méthode qui permet de mettre en évidence la maîtrise de la qualité de l'air dans la salle d'opération. Il s'agit de la simulation numérique en trois dimensions : Computational Fluid Dynamics (CFD). Cette solution permet de répondre aux attentes de maîtrise des risques en amont. Elle permet d'évaluer le comportement aéraulique de salles d'opération et de visualiser le flux d'air en intégrant les données réelles de la salle étudiée (géométrie, volume, débit d'air extrait et soufflé, température, hygrométrie).

Nosocomial infections hold an important part in hospital infectious diseases. Constant attention must be paid to control the microbial environment in at risk areas, which are defined according to the variety of the profile of hospitalised patients as well as the practiced care. Oftenly, the potential mechanisms of infection related to the environment are still to be defined scientifically. One way to clarify these mechanisms is digital simulation in three dimensions. This solution may be held for the review of the conception of operating rooms. It allows to meeting the requirements of risk control. Aeraulic modelling aim, by analysing stream coherence, to control the contamination in the operating zone, by bringing out the dissemination of the contamination within the operating room. It will allow to understanding clearly, due to animations and to virtual 3D reality, the drainages of air, the distribution of temperature, and the zones of poor air distribution. The aim of our study is to study and to realize two basic models of operating rooms, being able to serve as a base on the conception and the restructuration of the operating theatre. According to these two basic models of operating rooms we modelled two propositions by room type:
Orthopedic surgery (hyper aseptic type, Zone 4 or ISO5):
• | A blowing ceiling with unidirectional flow; |
• | A blowing ceiling with low velocity air movement. |
Digestive or urologic surgery (aseptic type, Zone 3 or ISO7):
• | Turbulent flow; |
• | A blowing ceiling with low velocity air movement. |
Finally, a parametric study for the orthopedic room with a blowing ceiling with unidirectional flow, was carried out to evaluate the impact of a contamination source according to its position in the room. Another parametric study for the room of digestive or urologic surgery equipped with a turbulent flow was realized in order to study the influence of ventilation output on the contamination of the operating zone.
