Nowadays, two distinct conceptualizations are available to classify, label and guide the treatment of psychiatric disorders: the diagnostic approach and the transdiagnostic approach. There are pros and cons to both approaches. We propose here to link these two conceptualizations by creating a two-level clinical model that takes advantages of both approaches, named the bifocal model (BFM). This two-tier clinical model consists of a double level of analysis: the first step is to identify transdiagnostic mechanisms involved in multiple disorders and then to recognize specific mechanisms identified in a given disorder or cluster of symptoms. Such a process would bring the diagnostic and transdiagnostic approaches together and offer a more flexible way to understand mental disorders and ultimately to improve medical outcomes.

Deux conceptualisations distinctes sont disponibles pour classifier et guider le traitement des troubles psychiatriques : l’approche diagnostique et l’approche transdiagnostique. Ces deux approches présentent des avantages et des inconvénients. Nous proposons de relier ces deux conceptualisations en créant un modèle clinique à deux niveaux qui tire parti des deux approches, appelé modèle bi-focal (BFM). Ce modèle clinique consiste en un double niveau d’analyse : la première étape consiste à identifier les mécanismes transdiagnostiques impliqués dans de multiples troubles, puis à repérer les mécanismes spécifiques identifiés dans un trouble ou un cluster de symptômes donné. Un tel processus réunirait les approches diagnostiques et transdiagnostiques et offrirait une manière plus souple de comprendre les troubles psychiatriques et d’améliorer la prise en charge.
