Résumé |
La chirurgie, de même que la radiothérapie exclusive ou les associations radiochirurgicales permettent d'obtenir des taux de guérison élevés, de 90 à 95 % lors du traitement des stades IB1 des cancers du col de l'utérus. Cependant, la radiothérapie externe associée à la curiethérapie, traitement standard des formes évoluées, ne permet pas de contrôler la maladie clans 35 à 90 % des cas Aucun progrès significatif n'a été rapporté ces 20 dernières années avec un tel traitement. Dans une étude de phase III, l'addition du 5-FU à l'irradiation n'a pas démontré de bénéfice sauf dans un sous-groupe discutable de patientes (tumeurs supérieures à 5 cm, stades IB/IIA ou IIB avec atteinte proximale du paramètre). Une chimiothérapie utilisant de l'épirubicine, à la fois concomitante et adjuvante, a permis d'améliorer la survie sans récidive dans un autre essai randomisé malgré un taux de contrôle local inchangé à long terme. Après de nombreuses études de phase II, cinq essais randomisés associant une chimiothérapie contenant du cisplatine à une irradiation ont démontré une diminution du risque relatif de récidive comprise entre 40 et 60% avec un risque relatif de décès diminué de 30 à 50%. Ce bénéfice est moins évident en cas de stades III ou IV que pour les stades moins avancés associés à de mauvais facteurs de pronostic. Malgré une toxicité hématologique et digestive plus importante observée chez les patientes traitées par radiochimiothérapie, les complications à long terme semblent comparables dans les différents groupes. Ces résultats demandent à être confirmés au cours d'une plus longue surveillance. L'importance de la chimiothérapie associée à la curiethérapie demeure un sujet à explorer. Enfin, reste à définir le meilleur protocole de chimiothérapie.

Low-stage uterine cervix carcinoma can be treated by either surgery, radiation therapy or combined treatments with high cure rates ranging from 90 to 95% for stage IB1 tumors. However, the standard treatment, combining external beam plus intracavitary radiation, fails to control the progression of the disease in 35 to 90% of patients with locally advanced cervical cancer. No substantial improvements have been made in the treatment of these tumors in the past two decades. The addition ot concurrent 5-FU in a phase III study failed to improve the results in the overall patient population, but the five-year DFS was significantly better in a subset of patients (tumor > 5 cm and IB/IIA or medial parametrial IIB disease). Concurrent chemoradiation and adjuvant chemotherapy with epirubicin showed, in a phase III study, a significantly longer DFS in patients treated with chemotherapy despite the same long-term local tumor control. After many phase II studies, five phase III studies have recently demonstrated a 40 to 60% reduction in the relative risk of recurrence with cisplatincontaining chemoracliation. Across these studies, the risk of death was reduced by 30 to 50%. The benefit was less clear in patients with stages III–IV tumors than in patients with lower stages associated with poor prognostic factors. Haematologic and gastrointestinal toxicity of chemoradiation was greater than that of radiotherapy alone. However, late side effects were similar in the different treatment groups. These results must be confirmed with a longer follow-up. The importance of concurrent chemotherapy during the brachytherapy procedure should be analyzed. It has yet to be determined which chemotherapy regimen achieves the most favorable therapeutic ratio.
